The following responses to his review comments are presented herein in the order as presented in his letter. I. A cul-de-sac can not be built, in terms of the physical size required. In lieu of a cul-de-sac being incorporated in the proposed roadway extension, the requirement that the Town of North Andover Fire Chief had regarding a fire sprinkler system installation in the proposed duplex dwelling has been accepted by the applicant, as so noted on the Title Sheet of the PIan Set. All other vehicles will be expected to use the proposed driveway to reverse direction on the proposed Methuen Avenue roadway extension. The abutter, Mr. Funaro, has expressed concern relative to the current paved way being used as a parking area during athletic events. As such, any further widening of the roadway could potentially invite more unwanted parking along the roadway during the events. 2. Should the Planning Board require that the existing portion of the paved way needs to be widened, to alleviate or prevent a condition where an unsafe situation may be created or coinpounded, or for any other health or safety concern, the applicant will agree to widen the roadway to 26' width, as noted-or otherwise required. Page 2 3. The previously submitted "List of Waivers Requested" a written document has been revised to include a request that a waiver to "Section 6.11 SIDEWALKS of the Subdivision Rules and Regulations be granted, due to short length of roadway extension involved, and that there are no existing sidewalks within the existing 40' wide private way nor along the northerly side of Sutton Street. 4. Soil testing for determination of the estimated seasonal high water table has been completed. This information has been used in determining the dimensions required for the three (3) proposed redesigned detention basins. 5. The revised plan now shows a 6"D.I.C.L. water main extension within Methuen Avenue, with a proposed blow-off valve at the roadway terminus. 6. This area serves as part of the existing driveway for#410 Sutton Street. The applicant prefers not to impose upon or alter any portion of this existing driveway. 7. The existing lot consists of two (2) lots based on the 1925 subdivision plan on record. The current lot as configured was established prior to 1975 (per Book 1243 and Page 116 for deed). A lot layout with dimensions and easements is shown on the revised Title Sheet of the Plan Set. 8. For this particular 100' road extension, where small flows will be generated, an open drainage system consisting of small vegetated or rip- rapped basins and swales is proposed, as opposed to traditional closed drainage facilitates with pipes and structures, as typically found on larger new-subdivisions projects. 9. These existing and proposed underground utilities are typically coordinated between the contractor and the individual electric, CATV, and telephone utilities companies, prior to start of construction. 10. The graphic scale and plan scale has been revised or corrected, as noted. 11. A 60' width for Sutton Street has been indicated, in place of 40', as noted erroneously. The following responses to their review comments for the subject project are presented herein in the order as presented in their letter, SECTION V—DEFINITIVE SUBDIVISION PLAN 5.2—For the existing lot, a separate lot plan is provided herein on Sheet 1, as requested, to clearly depict all bearings and distances for the existing lot boundary lines as well as the proposed drainage easements within the lot. The profile has been relocated fi-om Sheet 3 to Sheet 2 of the plan set, as requested. 5.2.2—A revision block has been added to each sheet of the plan set, as requested. 5.2.3 —Although indicated in Note 91 on the title sheet, the current property owners' name and address has been added to the title block, as requested. 5.2.5.2.1 —This information has been added. Refer to Item 5.2 above. 5.2,5,2.3 —There are already three (3) existing permanent monuments (one stone bound and two (2) iron pipes, as shown on the plan). 5.2.5,2.5 —The plan has been revised to show the change in lot area from 0,32 acres to 0.318 acres for the existing lot. 5.2.5.2.9—The ROW has been changed from 40' to 60', and the pavement width (32) has been added for actual dimensions of Sutton Street, as requested. I 5.2.5.3.8 —To the best of our knowledge, all of the surrounding residences are serviced by town water and sewerage systems, 5.2.5.3.10---Test pits for soil and water table determination have been undertaken. Results of these tests and design modifications associated with the field data submitted for review and approval. 5.2.5.115 —The existing benchmark is the rim elevation of an existing sewer manhole in Sutton Street. This information has been used to establish the elevation of the top of the existing stone bound located at the southerly corner of Sutton Street and Methuen Avenue and at the existing sewer manhole located at the intersection of Sutton Street and Methuen Avenue, which should serve as benchmarks during the construction layout process. 5.2.5.4.a—The plan is drawn at 20 scale instead of 40 scale, to clearly show the scope of the work within the existing Methuen Avenue right-of-way, and within the existing 13,850 square foot lot in a more detailed and comprehensible manner. As you may recall, a design for the Reilly site on Johnson Street was redrawn from 40 scale to 20 scale, as requested, solely for clarification purposes, acceptable to the Board. 5.2.5.4.b.1 —The bearing and distance of the construction baseline in the Methuen Avenue layout has been added to the plan, as requested, 5.2.5.4.b.6—These bearings and distances have been added to the plan. See response to Item 5.2. Also, a drainage easement should not be required within the existing private way. 5.2.5.4.b.7--Dimensions with off-sets have been added, as requested, for these proposed pavement surface areas. 5.2.5.4.b.10--The proposed 1 112 "copper water service has been deleted, and a 6"D.I.C.L water main with a 6" gate valve at the proposed 12"x 12"X 6" tapping sleeve in now proposed. 5.2.5.4.b.12—A gas service from Sutton Street to the dwelling will be coordinated by the local gas provider, and has been shown on the revised plan schematically. MERRIMACK ENGINEERING SERVICES,INC. 66 PARK STREET ANDOVER,MASSACHUSET'TS 01810 j 5 2.5.4.b.13 ----Electric telephone and cable services will be extended from the existing service lines in Sutton Street through underground PVC conduits to the proposed duplex dwelling, per the individual utility company standards as applicable. 5.2.5.4.b.14—Refer to our response to Item 5.2.5.3.15 i 5.2.5A.c.7--The proposed water service has been revised. A 6" ductile iron water main will be installed in Methuen Avenue, as required by Jack Sullivan, Director of Engineering at the Division of Public Works. This water main will have 5' of cover, as depicted on the revised profile. 5.2.5.4.c.8—The proposed sewer forcemain from the dwelling to the proposed sewer manhole in Methuen Avenue is now shown, in part, on the revised profile. The sewer manhole is shown at STA 11+00 on the profile and plan view as intended. 5.2.5.4.c.9—The vertical separation between the existing gas main and proposed sewer is shown on the profile, and the location of this item is shown on the plan. 5.2.5.4.c.10—The roadway centerline in Methuen Avenue has been marked or staked, as appropriate. 5.2.5.5.a.1 The proposed roadway will be widened from 23' —0"+/- to 26', as per the plan, and as noted on the revised typical section. a2—The typical section, in this situation, has been revised accordingly, to show the proposed sewer main to be installed approximately four(4) feet from the centerline. a3 —A 6" D.I.C.L. waterman has been added, to also address a DPW review comment. a4—In this situation, the cover over the proposed sewer extension will be at least 5.3' and a maximum of 8.5' in Sutton Street, as shown on the profile. a5—These utilities will be installed per the detail—typical section as revised, or as otherwise acceptable to the DPW and the respective utility company. MERRIMACK ENGINEERING SERVICES,INC. 66 PARK STREET ANDOVER,MASSACHUSETfS 01810 � ' 1 a6—The anticipated limit of grading will be up to the existing property line of the abutting property owners on Methuen Avenue. a7—The typical section has been revised to depict the "No Sidewalk" detail. 1 a8—Although not indicated on the current town standard profile, the depth of gravel borrow material shall be 8", as indicated under `Pavernent Notes' below, and has been added to the profile, as well. i a9—The 4" binder notation has been changed to 3"on the plan. a10—The notation in the plan and profile, "PROP EARTH BERM WITH 2:1 SLOPES", has been revised to include the work RIP-RAP, for lower maintenance and greater stabilization of the earth berm. The 2:1 rip-rap will taper to 3:1 sideslope, where the transition from rip-rap to grass will occur. 5.2.5.5.b,1 —Typical cross-section for the two (2) proposed detention basins have been included on the revised detail sheet. b2 Labeling has been revised for consistency between the detail and plan labels at the outflow structures. b3 —The 100 year peak outflow elevation, as revised, is shown on the detail, fi•om 138.12-to 138.42 at detention basin P-2 , in the rear of the lot. 5.2.5.5.c—The swales will be built to the detail shown; with respect to the existing grade, a 6" deep channel will be constructed. An actual profile would not serve to benefit the contractors and is subject to field-determination deviation (meander) during construction. 5.2.6.d—Lot closure calculations for the existing lot and roadway (Methuen Avenue—40' wide private way) are provided herein. SECTION VI—REQUIREMENTS FOR IMPROVEMENTS AND DESIGN 6.2.1 —Existing trees as shown will be removed only as necessary to allow for the work to be accomplished as proposed or approved. Trees wells or rings as necessary or practical to preserve and protect any remaining trees, as many as possible, will be emphasized prior to and during construction, and as may be stipulated by the Board as a condition of approval. 4 MERRIMACK ENGINEERING SERVICES,INC. • 66 PARK STREET ANDOVER,MASSACHUSETfS €11810 6.3.3 —Areas for temporary soil stocking, as well as erosion control devices along the limit of proposed work have been added to the plan. During construction the proposed stormwater detention areas will be excavated and used as sedimentation basins on a temporary basis as required until the site is stabilized against erosion. Additional notes have been added to the plan, as well as a separate written report entitled "Erosion and Sedimentation Control Plan", 6.3.4--A grass swale is proposed which will intercept and divert runoff away from the rear of the Ottavianni property, and toward a small basil)proposed at the lower area at the westerly corner of the lot, 6.8.7—No response required. 6.13.2--The `identification tape' notation has been added to the trench detail shown on ,Sheet 4 of the plan set. 6.13.3.11—No response required. 6.13.3.IV—No response required 6.14.5 —Stormwater will overtop the outflow v-notch weir and surrounding berm should flow exceed the projected 100 year flow rate. The berm at basin P-2 will then function as a level spreader device during that event. At basin P-1, an emergency overflow rip-rap spillway has been added to the plan and detail sheet. 6.14.9—Refer to our response to Item 5.2.5.3.10 relative to test pit data to ascertain groundwater(estimated seasonal high water table) for the proposed basins. 6.18.1 --There are several existing trees at or near the street and proposed roadway extension which shall be preserved and protected, in lieu of planting new trees; or as otherwise directed by the Planning Department or Tree Warden. 5 MERRIMACK ENGINEERING SERVICES,INC, 66 PARK STREET ANDOVER,MASSACHUSETTS 01810 �� i APPENDIX V—RULES AND REGULATIONS GOVERNING 1 STORMWATER MANAGEMENT 3.B.4—Soil tests to ascertain seasonal high groundwater levels have been undertaken and the results are submitted herein for review and for comparison and, as necessary, modification to the proposed basins in three (3) locations on the subject lot. 3.F.2.ii—A detailed plan outlining maintenance schedule and responsibilities, in an"Operations and Maintenance Plan", has been prepared and is submitted herewith for review and approval. 3.F.2.iii—A calculation for pollutant removal, or"Total Suspended Solid Removal Rate"has been computed and is submitted herewith for review and approval. 3.G—A narrative regarding nitrogen and phosphorous removal from surface runoff has been prepared and is submitted herewith for review and approval. 5.0—The data from soil tests has been used to ascertain the elevation of the proposed bottom elevation of the three (3) proposed basins, 5.i---The implementation of the proposed drainage system of basins and swales, as designed, will serve to retain and detain surface runoff such that the abutting property owners will not be adversely affected by otherwise potentially excessive peak rates of runoff due to development of this residential house lot with a 100' long road extension. GENERAL COMMENTS a. A"roof drain detail"had been added to the detail sheet. b. The scale in the title block on sheet 2—Grading and Utilities has been revised accordingly. c. The"Bit.Cone. Paved Swale"detail has been modified accordingly. d. The labels relative to the proposed week guardrail have been modified accordingly. This detail has been modified accordingly, g. Sawcutting of pavement where proposed excavation is required for installation of utilities as shown varies in width, according or in proportion to the depth of burial of each utility or service. Therefore, the location and width of sawcut lines will be left to the discretion of the contractor. h. The potential of future development has been reviewed and it has been acknowledged by the concerned parties that no further extension of the Methuen Avenue roadway would be required or permissible, as there are wetland issues and, in fact, no need or expressed concern of any abutters or persons of interest in the extension of the roadway beyond this current proposal. i. Although the actual architectural plans for the proposed duplex dwelling have not been completed, it is not anticipated that the building height will be 35 feet or higher, exclusive of chimneys and cupolas. j. Each unit will have a two car garage (equivalent to four covered spaces) to satisfy the off-street parking requirements. DRAINAGE COMMENTS a. The actual on-site survey data was used in conjunction with the U.S.G.S. Map to determine the subcatchments and drainage flow direction. There is continuity and consistency in the methodology used in both pre and post development analysis. b. The hydrograph range from 5 ---20 hours is sufficient for depiction of the rainfall runoff rates during the storm event, especially where pertinent to a small watershed. Any flow prior to or beyond the given time span is deemed irrelevant to the results and purpose of the drainage analysis, c. The drainage areas have been recalculated, Refer to the revised drainage analysis and drainage subcatchment plans for these revisions. 7 MERRIMACK ENGINEERING SERVICES,INC. 66 PARK STREET ANDOVER,MASSACHUSETTS 01810 d. There is a slight conservatism shown in the analysis which includes a small amount of extra area contributing area of runoff. e. The infiltration rate during the summer months increases when water tables are generally lower than the estimated seasonal high,water table. However, given the field-verified estimated seasonal high water table, as verified by actual test pits, the revised drainage analysis excludes exfiltration from ponds P-1 and P-2. f. The proposed slope of 0.012 1/1 was established based on the following calculations; 138.7— 137.5/100' =0.012 1/1 (Refer to Item g below) g. A 40' long grass swale at s —0.008%will be included in the revised drainage analysis. h. The revised drainage analysis reflects a 110 degree V-notch weir, with a 2' width at invert. The surrounding berm has been raised to reflect the resulting higher ponding elevation drainage peak detention period as determined in the revised drainage analysis. i. The notation"El = 141.3"has been removed from the detail, as noted. Given all of the above, please feel free to contact me should you have any questions or comments regarding our responses and the significant modifications to the revised plans and drainage analysis enclosed herewith. General/Drainage Comments 1, The Post-Development drainage analysis has been revised to reflect a correction which includes the 1000 square foot discrepancy as noted, in SubcatclTment#2. 2. A revised subcatchnient plan at full scale (1"=20') is included for review. 3. The revised subcatchmant plan(noted in Item#2 above) now includes the flow arrows for general direction of overland flow through the individual subcatchments. 4, A numerical breakdown of the computed storage volumes available within the recharge/detention facility for roof runoff only is included, with associated `Cumulative' volume calculations, for review. 5. The proposed blow-off valve on the end of the watermain is acceptable and preferred by Mr. John Sullivan,DPW Director of Engineering. 6. This detail is now current with the Town standard specification as detailed in the Subdivision Rules and Regulations, issue date November, 2000, The roadway pavement base course thickness is acceptable at 3". The subbase material has been revised flour 8" to 12"on the detail, in compliance with Section A.5 of Appendix 1. Given all of the above, please review the enclosed documents as revised. In general, the maintenance of the proposed roadway, utilities, drainage swales and stormwater detention area will be solely the responsibility of the homeowners of the duplex dwelling. Therefore only four(4) waivers are now requested.
