The North Andover Planning Board has voted to APPROVE said plan, subject to the
following conditions:
1. That the record owners of the subject land forthwith execute and record
a "covenant running with the land", or otherwise provide security for the con-
struction of ways and the installation of municipal services within said sub-
division, all as provided by G.L. c. 41l S. 81-U.
2. That all such construction and installations shall in all respects
conform to the governing rules and regulations of this Board.
3. That, as required by the North Andover Board of Health in its report to
this Board, no building or other structure shall be built or placed upon Lots
No. as shown on said Plan without the prior
consent of said Board of Health.

This decision supersedes the original decision to disapprove
this subdivision issued by the Planning .Board on June 18, 1986.
In the event that no appeal shall have been taken from said approval within
twenty days from this date, the North Andover Planning Board will forthwith
thereafter endorse its formal approval upon said plan.

The Planning Board voted to approve the above ubd ' vi5ifz� �$a
with the following conditions : TE8 '
1 . The curb-cut shown for Ledgeview Drive on Route 114 and all
site improvements for the subdivision shall receive approval
by .the State DPW.
2 . The 50 ' wide greenbelt adjacent to Route 114 shall be labeled
as a "no-cut zone" and should be left in its natural condition.
Lot 1 should also indicate a 50 ' wide greenbelt. Clearing
necessary for sight lines at the intersection of Ledgeview
Drive and Route 114 will be allowed.
3 . A note shall be placed on Lot 5 which shall state that if
the lot does not obtain Board of Health approval for septic
design by Duly 1 , 1988 , the lot will be dedicated as open
space in a manner acceptable to the Planning Board.
4 . All public access easements in this subdivision shall be
filed with the Registry of Deeds Office . A deed restriction
to allow public access on the lots as indicated in the
subdivision plan. These easements shall be marked by
stone markers located at the edge of the easement adjacent
to the right-of-way of Ledgeview Drive and shall attach a
marker which contains the following language, "Public
Access to harold Parker State Forest. "
5 . All lots shall receive the necessary approvals by the
Board of Health .
6 . The Planning Board grants a waiver for the cut.-de-sac .
(500 ' regulations approximately 860 ' shown)
7 . All drainage easements indicated on the plan shall be
properly filed with the Registry of Deeds office .
8 . The subdivision will tie into the Town ' s water system
in accordance with the requirements of the Town of
North Andover DPW.
9 . All hydrants branches shall be provided with six inch
Mueller or equal gate valves (open left) . All hydrants
shall be Gddy Breakflange hydrants to North Andover
specifications .
10 . Tree cutting shall be kept to a minimum throughout the
project in order to -minimize erosion and preserve the natural
features of the site accordingly, the developer , in
conjunction with a registered arborist shall submit a tree
cutting and reforestation plan (the plan is for areas of
substantial cutting and filling) to be reviewed by the
Planning Board. - This plan shall explicity include specific
trees to be retained . ALso the developer shall inform the
'.Town Planner when significant tree cutting is to occur in
order that the Planner may determine the need to be present.

11 . All Planning Board Order of Conditions are to be placed on
the recorded definitive plan, (cover sheet) prior to signature
at a Planning Board Meeting .
12 . Prior .to signing the definitive plans all executed deeds of
easements and parcels shall be submitted to the ,Planning
Board and held in escrow until completion. At the time of
completion and prior to final security release , the developer
shall execute such documents for the Town .
13 . A certified document. shall be submitted to the Planning
Board after the completion of each phase verifying that
all utilities (including electric , telephone, sewer , and cable)
have been installed prior to binder coat of application.
Installation shall be in accordance with plans and profile
prior to the release of any lots .
14 . The 100 year flood elevation line shall be placed upon
the subdivision plans .
