DESCRIPTION OF VIOLATION
Fail Code
OBSERVATION: After running multiple cycles, observed high temperature dish
machine not achieve a 160°F utensil surface temperature via PIC's 160OF
sensor stickers. Observed dish machine display final rinse temperatures
between 94-114 .2°F. PIC to call a repair service if needed and provide
the health office with a status or repair slip within 24 hours. PIC to
use the three compartment sink for sanitizing food-contact surfaces until
dish machine is in good repair and achieving a utensil surface
temperature of 160OF or above.

Sanitizing Hot Water and Chemical-Methods
Discussion Observed packages of sushi for sale in open face refrigerator
from "Pick N Roll Sushi" in Boston, MA. PIC states no sushi is made on
site. Observed consumer advisory on packaging in compliance. PIC states
no other animal foods are served raw or undercooked.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action

In Compliance Observed digital copy of the employee illness agreement
document available. Observed labeled bucket of sanitizer measure 1875 ppm
lactic acid via PIC's test strips. Observed sliced turkey in the top rail
of the sandwich in line refrigerator at 38°F; yogurt in the drawer of the
smoothie in line refrigerator at 41°F; open face retail refrigerator
holding non-TCS drinks and sushi operating at 35°F; grilled chicken in
the "Continental" refrigerator at 33°F; grilled chicken that date marking
and PIC state was cooked approximately 3 hours ago at 39°F in the walk in
refrigerator.
