Every application for action by the Board hall be made on a form
approved by the Board. These forms shall be furnished by the
Town Clerk upon request. Any communication purporting to be an
application shall be treated .as mere notice of intention to seek
relief until. such time as it is\ made on the official application

with the requirements of Section 8. 3 .

APPLICATION'FOR SPECIAL PERMIT

The present use of the duplex house on Park Street is storage and employee meetings.
It is our hope to be able to locate space for staff ..on the first floor of the house.
Emile Guy, Chief Project Engineer for the Commonwealth of Massachusetts has given
his permission for this project as it is the least expensive way to generate the
needed space. A copy of the recap letter is attached.
The office space that we need to develop will be used for clerical support staff and
therapists. We do not want any of the space for direct patient services.
At this writing, the building is used primarily for the storage of equipment. This
is not the most effective use of very expensive space. Thei: permission that we are
looking for requires no structural changes inside or outside. Presently, space is very
limited inside our nursing facility. Some departments are so cramped for space that
they cannot provide effective patient support..
By allowing us to locate some office space inside the empty house, we can spread out
to make better use of the present space inside the nursing facility.

RE: Facility's Business Office Location and Function
Dear Emile:
Thank you for meeting with me yesterday to review proposed remodeling projects.
As I mentioned to you, some of our clients are proposing to use buildings adjacent
to their existing facility to supplement their business office.
Based on our conversation on the subject, it is my understanding that the
Department of Public Health requires direct access to the Business Office for both
residents and the general public and that e Business Office be located within the
facility. Furthermore, all records per' Ai rig to current residents shall be located
within the facility and accessible to the Business Office.
It is also my understanding that support staff of the Business Office and records of
deceased residents may be annexed outside of the facility. Also, relocation of
functional areas within the facility must be approved through DHCQ prior to any
changes taking place.
If I'm in error of the Department of Public Health Interpretation on this subject,

Rehabilitation Properties Trust for special permit
concerning 59-61 and 75 Park and 290 Osgood Streets
Dear Board Members:
This letter is to clarify the relief sought by petitioners.
SLF and HRPT propose to extend the existing nursing home use of 75
Park Street onto two adjacent parcels: 290 Osgood and 59-61 Park.
Specifically, they propose to (1) construct an accessory parking
lot serving the nursing home to be located primarily on the 290
Osgood parcel and (2) develop an outdoor sitting and walking area
for nursing home residents to be located primarily at 59-61 Park.
if the Board approves of these extensions, the petitioners
request that a special permit authorizing the nursing home use as
extended be issued. SLF and HRPT do not, however, waive their
position that the existing nursing home use at 75 Park Street is a
valid nonconforming use.

In accordance with the provisions of Chapter 40-A, Section 11,
M.G.L. , the North Andover Planning Board will hold a public hearing
on Tuesday evening, December b, 1994 at 8 : 00 p.m. in the Senior
Center behind the Town Building, on the application of the Greenery
Extended Care Center, 75 Park Street, North Andover, Ma. , for a
special permit under Section 8 . 3 (Site Plan Review) of the North
Andover Zoning Bylaw to review the proposed usage of office space
and storage on property located at 59-61 Park Street.
A copy of the application and plan may be inspected at the
North Andover Planning Board Office during the hours of 8: 30 - 4 : 30
Monday through Friday.
