We had permission to do so by the head nurse on duty at the time. The videos sent to you were AFTER the pest control had been done 24 hours prior. The ants were being found in/on the bed as well. We (my family and 1)will continue to take whatever steps are necessary to prevent the ant problem should it continue, The facility did nothing but call the pest control -which took 5 days to come to my room. Their actions were not effective and the condition worsened. am disappointed that you did not come to my room to speak with me regarding the complaint. You spoke only to management. Not speaking with me suggests that there is some bias, and the investigation incomplete. Room 129 next door to me is also ant infested as shown in the video sent to you previously, As for the food temperature issue. I would encourage you to come to my room and test the temperature of the hot food that is served to me. I will be purchasing a food temperature thermometer and will be testing my meals when they arrive and video documenting the temperature(s).Again, you are relying on past inspections and not current conditions. Based on this new complaint you should come out and re-Inspect the food temperatures at the point of delivery. It should be a minimum of 135 degrees Fahrenheit by code. The Massachusetts Department of Public Health was informed of my concerns in detail during their recent visit, The final report has not yet been published. Additionally, you are partially correct regarding food selection, however; I am never given the choice of what to have for breakfast. I get whatever comes on the tray. As for lunch and dinner; I am offered a main dish or an alternate dish. I am presented with a sheet outlining the days menu around 8:30 each morning. That being said -what you order and what actually arrives at meal times can be quite different.The side items are almost always different, the main item is often a variation of what was described. The menu and food service in general are completely lacking in any consistency and quality control. Substitutions without notification are the norm. Hot items served cold is the norm. During my stay here I have personally spent over$1000 on food and my family brings items as well. There are regular occasions where I have to order food delivered from outside sources just to have a proper meal. Any food items that I have in my room are snacks that are single servings and factory sealed in their original packaging. There is no open food left around the room. I do realize that food safety aside, the abysmal food service operation here is largely outside of your control. It's truly sad and unfortunate that management places zero priority on the quality of food service here at Andover Forest. It's no coincidence that when the Mass. DPH inspectors were on-site there was a temporary marked improvement in the food quality. Once the inspectors were gone, the food went right back to the way it always is. would hope that you would continue your investigative efforts and actually come to test the temperature of the hot food when it arrives to my room for yourself, however- I will test the temperatures myself and keep a detailed log and videos of the results, which I will forward to you weekly. appreciate your time and ask that you forward me copies of any and all reports generated as a result of my complaints under the Massachusetts Public Records Law, Mass. They are having pest control come by again this week. We ask that you do not spray for ants yourself or do any of your own pest control. Often, other chemicals will counteract what pest control is doing and the issue will not get resolved if this is happening. We also need to make sure that any food in your room is placed in containers and kept off the ground. We need to eliminate any food sources for the ants. As far as food temps are concerned we, the town, did an inspection at the end of January and did cite or notice any issues that pertain to food temps or food safety. The state was also in recently for their inspection. They did not cite or find any violations when it comes to food safety.This is something that is important to all of us and we work together to make sure that food is being served in a safe environment. In terms of food substitutions are concerned, it is my understanding that they ask at each meal what you would like. If there are any allergy concerns please let the staff know. It was explained to me by Jennifer that sometimes there are issues with deliveries and some of the food items are not available last minute due to these issues.This is something that they seem to be doing their best at but some of the issues are out of their control. I will keep in contact with Kalie and Jennifer going forward to make sure we are all on the same page. Just a quick further update, on Saturday a family member went to Home Depot and purchased "ant cups" and "Raid Ant Spray"and we self-treated the perimeter of the room. We also discovered that the room next door to me (129) is also infested.We did not treat the room next door. First Concern: Pest Control Issue My room has developed an ant infestation that was formally reported to management on Monday, March 31, 2025. Despite bringing this matter to the attention of multiple staff members, including the housekeeping manager(who stated this was not their department's responsibility)and the facility maintenance manager (who acknowledged the issue and indicated he would contact pest control), no remedial action has been taken as of today's date. Second Concern; Food Service Deficiencies The facility's food service operation consistently fails to maintain appropriate food temperatures. Hot food items are routinely served cold, well below proper serving temperature. This appears to result from inadequately heated warming equipment and extended holding times prior to distribution to residents. Additionally, menu items are frequently substituted without prior notification, raising serious concerns about potential exposure to allergens. I respectfully request prompt investigation and resolution of these issues.
