242-880- COC Request Lot 3 Turnpike Street(Gunva Corp.)(WPI)
This Order was issued on 9/18/97 authorizing the construction of a single family dwelling,
septic system and associated appurtenances within the Buffer Zone to a BVW.
The Applicant (s) submitted revised As-Builts On a motion by Ms Feltovic, seconded by
Mr Lynch, the comnnssion voted unanimously to continue to 6/20/01

This Order was issued on 9/18/97 authorizing the construction of a single family

This Order was issued on 3/19/98 authorizing roadway, traffic and safety
improvements at the Marston Street/495 Interchange. Portions of the
construction include realignment and paving of I-495 and
acceleration/deceleration ramps,median re-construction,and stormwater
mitigation. The Applicant(s)have submutted proof of certified mailing On a motion by
Mr Lynch, seconded by Ms Feltovic,the commission voted unanimously to accept the
Extension for Order of Conditions as drafted

This Order was issued on 10/4/00 as a result of a violation notice, enforcement
order and a fine. The Order was issued for a septic tank repair at 110 Laconia Cr.
The Order was issued after the work was completed. The Applicant(s) paid the
fine. On a motion by Mr.Lynch, seconded by Ms Feltovic, the commission voted unanimously
to grant the request of the Certificate of Compliance as drafted

` This Order was issued on 10/22/98 for the installation of a sewer line and water
line associated with the construction of a single family house outside the 100'

This Order was issued and amended on 6/16/99 authorizing the construction Abbott
Village Subdivision within the Buffer Zone of a BVW On a motion by Mr Lynch,
seconded by Ms Feltovic, the commission voted unanimously to grant the request of the
Certificate of Compliance as drafted upon receipt of revised plans

200 Chickering Road(Wetlands Preservation,Inc) cont from 5/16/
This RDA is for the wetland boundary confirmation of the resource areas on
Wood Road (paper street) which is adjacent to the Rennies property and for an
isolated wetland on the Rennies property not previously delineated. This
delineation is in addition to and not in place of the existing resource areas
previously confirmed through DEP#242-856. On a motion by Mr Manzi, seconded by
Ms Feltovic, the commission voted unanimously that it was positive

242-1065-Boxford St. 104D Town of North Andover(Ben Gary)
This NOI is for the construction of three athletic fields, gravel driveway,paved
entryway,gravel parking lot, fencing,well,water main, irrigation system and
leaching basins within the buffer zone a BVW. I have spoken with the
representative for this project in regards to additions to the plans and items
needed in the field.
This was continued to the 6/20/01 meeting
242-1063- 544 Foster St.Mike Olsen (Advantage Real Estates) (BDO Eng.)
This NOI is for the replacement of a septic system in the buffer zone to a BVW, Inner
Riparian Zone to Rocky Brook and flood plain. This is a very restricted site and the
proposed limit of work is outside the 25' NDZ. The leaching field is proposed 60' from
WF-1 and 63' from WF-10. A motion was made by Mr Manzi, seconded by Ms Feltovic, to close
the public hearing and issue a decision within 21 days The vote was unanimous
242-1078-350 Summer Street(Hancock Environmental)
This NOI is for the installation of an inground swimming pool, associated sheds,
fence, landscaping, grading and an area of the 25' NDZ that is to be revegetated.
Abutter's Mr &Ms Trombly represented by Kurt Young voiced their concerns over water
flow on their planting beds. Both parties are seeking a resolution with further study. On a
motion by, Mr Manzi, seconded by Ms Feltovic, the commission voted unanimously to
continue to 6/20/01

This NOI is for the installation of a 16'X24' above ground pool within the Buffer
Zone to a BVW. The Applicant(s) will submitted revised plans On a motion by, Mr
Manzi, seconded by Ms Feltovic, the commission voted unanimously to continue to 6/20/01

This NOI is for the installation of an 8" sewer main and 8"water main in the shoulder of
Rt. 125 within the 100' Buffer Zone of a BVW. This NOI is filed in addition to the filing for
the restaurant previously approved under DEP#242-1036. The Applicant(s) will submit
revised plans A motion was made by Mr Manzi, seconded by Ms Feltovic,to close the public
hearing and issue a decision within 21 days The vote was unanimous
