General Business: 242-880-COC Request Lot 3 Turnpike Street(Gunya Corp)(WPI) This Order was issued on 9/18/97 authorizing the construction of a single family dwelling, septic system and associated appurtenances within the Buffer Zone to a BVW Mr LaGrasse recommended denying the COC because the Applicant(s) have requested more than one continuance and have not submitted any more information On a motion by Mr Manzi, seconded by Mr Lynch,the commission voted unanimously to deny the request for continuance 242-881 -COC Request Lot 2 Turnpike Street(Gunya Corp)(WP1) This Order was issued on 9/18/97 authorizing the construction of a single fainily dwelling, septic system and associated appurtenances within the Buffer Zone to a BVW Mr LaGrasse recommended denying the COC because the Applicant(s)have requested more than one continuance and have not submitted any more information On a motion by Mr Manzi, seconded by Mr Lynch, the comrrussion voted unanimously to deny the request for continuance 242-983-COC Request 10 Summer Street(Came Kirby) ~` This Order was issued on 10/6/99 authorizing the construction of a single fammly dwelling,driveway, grading,utilities, sewer connection and associated appurtenances within the Buffer Zone to a BVW and Outer Riparian Zone to Mosquito Brook Mr LaGrasse stated the applicant(s)have submitted revised plans that are satisfactory On a motion by Mr Manzi, seconded by Mr Lynch,the commission voted unanimously to grant the request of the Certificate of Compliance as drafted 242-589-Order of Conditions Modification request Lot 12(Heritage Estates) This order was issued on 115100 authorizing the construction of a 12 Lot subdivision,roadway, sewer,utilities,drainage structures and associated appurtenances within the Buffer Zone of a BVW Mr LaGrasse recommended to issue a Modification for Lot 12 Heritage Estates and accept plans dated 6/6/01 REV 6/7/01 as the plan of record for this Lot On a motion by Mr Manzi, seconded by Mr Mansour, the commission voted unanimously to grant the request of the Certificate of Compliance as drafted 242-820—COC request Sterling Ln (Farr Better Homes) (Coolidge Construction) This order was issued on 10/16/96 authorizing the construction of a limited project roadway with a crossing, stormwater management,flood storage compensation,replication, and two single family dwellings with associated appurtenances within the Buffer Zone to a BVW and BVW alterations On a motion by Mr Manzi, seconded by Mr Lynch, the commission voted unanimously to grant the request of the Certificate of Compliance as drafted 242-922—Order of Conditions Extension request Lot 20A Bridle Path This order was issued on 9/3/98 authorizing the construction of a single family dwelling within the Buffer Zone to a BVW Contingent upon the re-establishment of 14 flags On a motion by Mr Manzi, seconded by Mr Lynch,the commission voted unanimously to grant a continuance to the August V meeting 8 00 P M Public Hearings RDA's Chestnut St/Rea St (Epsilon Assoc ) Mr LaGrasse commented that he would like to either walk the wetland boundary line with Epsilon to re-establish the missing flags in the field,or have them done by survey Mr LaGrasse also wanted the Title Block of the plans to be corrected from ANRAD to RDA On a motion by Mr Manzi, seconded by Mr Lynch, the commission voted unanimously that it was positive 796 Winter St (Gary Wilford)(NE Eng) This RDA is for a septic system repair within the Buffer Zone to a BVW The project is adjacent to Rocky Brook,but is exempt from the Riverfront Act(Lot created before 1996 and is a septic repair) The only work in the 100' Buffer Zone or Inner Riparian Zone is connecting to the existing septic tank and placement of the erosion controls On a motion by,Mr Manzi, seconded by Mr Mansour, the commission voted unanimously to continue to 7/18/01 Notice of Intent(s) 242-1020-492 Sutton St (MA Aeronautics Com)(Dufresne-Henry) A motion was made by Mr Manzi,seconded by Mr Mansour,to close the public hearing and issue a decision within 21 days The vote was unanimous 242-1058-Lot 13 Nutmeg Lane-map 38 parcel 284(No Andover Land Corp)(B Osgood,Jr) This NOI is for the demolition of an existing house and construction of a new dwelling within the 100' Buffer Zone to an isolated wetland They are proposing the project as a redevelopment project with a 15' NDZ and a 25' NCZ with a stone wail along their proposed NDZ On a motion by,Mr Manzi, seconded by Mr Lynch,the commssion voted unanimously to continue to 8/l/01 242-1065 -Boxford St 104D Town of North Andover(Ben Gary) This NOI is for the construction of three athletic fields, gravel driveway,paved entryway, gravel g a parking lot,fencing, well, water main,irrigation system and leaching basins within the Buffer Zone a BVW Mr LaGrasse has spoken with the representative for this project in regards to additions to the plans and items needed in the field On a motion by,Mr Manzi, seconded by Mr Lynch, the commission voted unanimously to continue to 9/5/01 242-1078- 350 Summer Street(Hancock Environmental) This NOI is for the installation of an mground swimming pool, associated sheds,fence, landscaping, grading and an area of the 25' NDZ that is to be revegetated Abutter's Mr &Ms Trombly represented by Kurt Young,WPI,voiced their concerns over water flow on their planting beds Julie Pamno,Hancock,representing the applicant, showed with pictures the Trombley's flower bed was a berm that had been put in with no filing or approval from the Commission Ms Pamno explained the applicants had restored the land in question to better condition than it was originally Both parties are seeking a resolution with further study The applicant will submit revised plans On a motion by,Mr Manzi,seconded by Mr Mansour, the commission voted unanimously to continue to 7/18/01 NACC-11 429 Waverly Rd (James Dufresne)(Rmght Now Realty Trust) This NOI is for a roadway,three houses, associated appurtenances,detention basin and stormwater system The applicant will submit revised plans with any additional structures needed On a motion by,Mr Manzi, seconded by Mr Lynch,the commission voted unanimously to continue to 8/1/01 242-1079— 1939 Great Pond Rd (Frank Ragonese) (Riverside Eng) This NOI is for the demolition of an existing house and construction of a single family dwelling, deck,utilities,roof run off infiltration system, and gravel drive within the Buffer Zone to a BVW Mr LaGrasse stated that the plan is difficult to read The line weights for various items are the same and it is hard to follow Mr LaGrasse asked the engineer to clear it up,but what had done was nummal He continued saying the plan states that it is a 20' scale and the distance from the B series wetland line to the house corner is noted as 55',He checked it and scaled it off as 60' Mr LaGrasse 0, f pointed out this needed to be corrected Mr LaGrasse recommended the Applicant submit revised plans with the corrections and continue to the 8/1/01 meeting Mr Lynch stated that there was clearly a problem with the mechanics of the site plan Seekamp Environmental will submit revised plans Mr Mansour questioned if the house was tied into the original water and sewer Paul Lawson, Abutter, spoke on behalf of Frank Ragonese saying Frank maintains his property well and has no issues with this project On a motion by, Mr Manzi, seconded by Mr Mansour, the commission voted unanimously to continue to 8/1/01 242-1081 -557 Boxford St (William Tompkins) This NOI is for the installation of a 16'X24' above ground pool within the Buffer Zone to a BVW Mr LaGrasse commented that the wetland flags were re-labeled and the driveway has been correctly drawn A motion was made by Mr Manzi, seconded by Mr Mansour, to close the public hearing and issue a decision within 21 days The vote was unanimous 242-1083 -7 Sullivan St (Kevin Travers)(NE Eng) This NOI is for the replacement of a septic tank and leaching field The new septic system will be installed in the same location as the existing system The system is proposed 72' from the existing wetland line It is stated on the plan that the wetland line was taken from a plan of record The applicant states that the wetland line was approved by the Commission under an RDA,but has since expired and needs to be re-delineated On a motion by, Mr Manzi, seconded by Mr Lynch,the commission voted unanimously to continue to 8/1/01 242-1086- 171 Brentwood Circle(Scott Giles) This NOI is for the construction of a 10'x12' screened in porch within the Buffer Zone to a BVW The wetland boundary is depicted at 72' from the proposed structure The erosion controls are proposed within the existing grass area and no clearing will be needed The proposed excavation is for two I'D sonotubes Mr LaGrasse asked to have the project re-labeled and the plan should be a 20' scale On a motion by,Mr Manzi, seconded by Mr Mansour,the commission voted unanimously to continue to 7/18/01 242-1085 -77 Bruin Hill (Scott Giles) This NOI is for the construction of a 24'x25' addition and driveway extension within the Buffer Zone to a BVW It states in the notice that the addition is proposed at grade and there will be no grading for the addition Mr LaGrasse was concerned with the plan is a 30' scale plan and should be a 20' scale,the proposed driveway grades,the soil stockpile location shown on the plan, and if there is going to be enough room between the haybale line and the proposed driveway extension for construction purposes On a motion by,Mr Manzi, seconded by Mr Mansour,the commission voted unanimously to continue to 8/15/01 242-1084-443 Boston St (John Mongell)(Scott Giles)tncomplete, will not be heard Decisions 242-1063 -544 Foster St Mike Olsen (Advantage Real Estates) (BDO Eng This NOI is for the replacement of a septic system in the Buffer Zone to a BVW,Inner Riparian Zone to Rocky Brook and flood plain This is a very restricted site and the proposed limit of work is outside the 25' NDZ The leaching field is proposed 60' from WF-1 and 63' from WF-10 On a motion by Mr Manzi, seconded by Mr Lynch,the comrrussion voted unanimously to approve the Order of Conditions as drafted and amended 242-1080- 1070 Osgood St (Chas Senabunyarithi)(Neve Assoc ) This NOI is for the installation of an 8" sewer main and 8"water main in the shoulder of Rt 125 within the 100' Buffer Zone of a BVW This NOI is filed in addition to the filing for the restaurant previously approved under DEP#242-1036 On a motion by Mr Manzi, seconded by Mr Lynch, the commission voted unanimously to approve the Order of Conditions as drafted and amended
