The North Andover Planning Board has voted to APPROVE said plan' subject to the`
following conditions:
1. That the record owners of the subject land forthwith execute and record
a "covenant running with the land' 9 or otherwise provide security for the con-11
struction of ways and the installation of :municipal services within said sub-
division9 all as provided by G.L. c. 41 , S. 81-U.

2. That all such construction and installations shall in all respects '
conform to the governing rules and regulations of this Board. Ii
3. That9 as required by the North Andover Board of Health in its report to
this Board9 no building or other structure shall be built or placed upon Lots
NO. as .shown on said Plan without the prior
consent of said Board of Health.
4. Other conditions:
See conditions listed on the attached sheet.
In the event that no appeal shall have been taken from said approval within
twenty days from this date9 the North Andover Planning Board will forthwith
thereafter endorse its formal approval upon said plan.

SALEM FOREST SUBDIVISION - APPROVED August 4, 1975
List of Conditions:
1. A covenant be executed by the developer to ensure that the design improvements
specified in the definitive plan are accomplished.
2. No building permits shall be issued until a Board of Health septic permit is issued
for each lot.
3. Water mains and fire hydrants shall be installed according to the Board of Public
Works specifications.

4. Gravel shoulders shall be provided in accordance with #1 on Mr. Cyr's letter datekl.
July 28, 1975 recommending 18 in. beyond the edge of the pavement.

5. Certain changes to the drainage plan shall be incorporated as follows:
a) additional pairs of catch basins shall be located on the proposed Granville
Lane at stations 25, 27 w 50, 33 cr1L 50 and 36 z-:,±-_ 50; that the drainage course
from the meadow in the center of the development be an open ditch from approximately
the railroad right-of-way to Granville Lane; that the elevation coming into this
ditch be controlled by a spillway at a controlled elevation (112).
b) all drainage courses within the entire subdivision shall be deeded to the Town
of North Andover as easements for Highway Department maintenance*
c) that the drain pipe under the road at approximately station 3 be 30 in. concrete*
d) that an additional drain pipe be located across Granville Lane at approximately
station 5.
e) slope granite curb shall be provided on the south side of the proposed road at
station 35+20 to station 38*50.
f) all catch basins shall be of 8 in. precast concrete with checkerboard frame and
grate and shall be located off the traveled roadway to maximize water collection.
