Re: Jerad Place III
Dear Mr. Nardella;
The Division of Public Works has reviewed the plans for the
proposed Jerad Place Phase III P.R.D. , prepared for Robert J.
Janusz, designed by Thomas E. Neve Associates Inc. , dated April 22,
1994 . We have the following comments:
1 . Two leaching catch basins should be installed at the low
point at the end of the cul-de-sac.
2 . The area on the property downstream from the catch basins
should be graded to accommodate excess runoff from
greater volume storm events.
3. A cape cod berm should be installed on the perimeter of
the cul-de-sac. The berm should start at station 7+61.
4 . The drainage design should include the use of dry wells
(to capture roof runoff) on all the lots .
5 . The proposed hydrant at the cul-de-sac should be
installed on the end of the water main and be located
near the Lot 14/15 property line.
6. A 12 inch water main should be installed off site on
Forest Street. The main should extend from its current
terminus on Forest Street to the intersection of Jerad
Place Road, a distance of approximately 390 feet.

RE: Conditions for Accepting Open Space -Jared III PRD.
Dear Mr. Grifoni:
Prior to the Conservation Commission's acceptance of the Open Space, the Open
Space Committee requests that the following be completed:
1, The dedicated open space shall be marked in the field with an
applicable number of permanent markers including but not limited to
pipes and/or stakes labeled accordingly.
2. All access points shall be marked with a sign stating"North Andover
Protected Open Space." The North Andover Conservation
Commission (NACC) has adopted a specific sign; please contact the
Conservation Administrator for details.
3. All debris,including but not limited to tires, trash, and barbed wire,
shall be removed from the dedicated open space and disposed of off-
site in an acceptable manner.
4. Where applicable, existing and/or recently constructed trails shall be
cleared and marked in accordance with the requirements of the North
Andover Trails Committee (NATO). Please contact the NATC
Chairman for specifics.

5. An"as-built' plan of the designated open space at a scale acceptable to
the NAOSC and Planning Board shall be submitted with a clear
depiction of the following features:
➢ Property Lines;
➢ Open Space Markers;
➢ Wetland resource areas;
➢ Topography at 2' intervals, and;
➢ trails w/ access points.
Upon completion of these above referenced tasks please schedule a site visit with
a representative of the NAOSC. The Planning Board will require a favorable
recommendation from this group within 21 days from the date of this site visit
prior to agreeing to accept the dedicated open space parcel.
