The undersigned wishes to record the accompanying plan and requests a
determination by said Board that approval by it under the Subdivision
Control Law is not required . The undersigned believes that such approval
is not required for the following reasons :
1 . The division of land shown on the accompanying plan is not a sub-
division because every lot shown thereon has the amount of frontage
required by the North Andover Zoning By Law and is on . a public way ,

REVISION OF CONVEYANCE OF EASEMENTS AND UTILITIES
Campbell Forest, LLC of 11 Old Boston Road, Tewksbury, Massachusetts for consideration of
less than $100.00, hereby grants, transfers and delivers unto the Town of North Andover, a
municipal corporation in Essex County, Massachusetts as follows:
A. The perpetual rights and easements to construct, inspect, repair, remove, replace,
operate and forever maintain(1) a sanitary sewer or sewers with any manholes,pipes, conduits
and other appurtenances; (2)pipes, conduits and their appurtenances for the conveyance of water
and (3) manholes, pipes, conduits and their appurtenances, and to do all other acts incidental to
the foregoing, including the right to pass along and over the land for the aforesaid purpose, in,
though and under the streets and ways, including Donna Drive and Joanne Drive and a portion of
land entitled"Right of Way `A' to be conveyed to the Town of North Andover" as shown on a
plan of land entitled "Definitive Subdivision Plan of Land in North Andover, Mass., Scale 1"
=200'." Campbell Forest,North Andover, Massachusetts recorded with the Essex North District
Registry of Deeds as Plan No. 12784. Referenced to said plan is made and said plan is
incorporated herein for a complete and detailed description of said roads. See Exhibit A for a
metes and bounds description of these premises.
B. The perpetual rights and easements to use for drainage purposes the parcels of land as
shown on Plan 12784 and as referenced in the Form M recorded at the Essex North District
Registry of Deeds at Book 4452, Page 59.
C. The perpetual rights and easements to use the parcels of land situated in the Campbell
Forest subdivision in said Town of North Andover for public sewer purposes and as shown on
the plan of land entitled Campbell Forest Subdivision Sewer in North Andover, Massachusetts,
Applicant MPG Realty Corp. dated April 2, 1998 as amended on May 13, 1998 prepared by
McKenzie Engineering Group, Inc. drawing title: Sewer Plan& Profile, said plan being recorded
herewith.
Said plan reflects the change from septic systems to public sewer of said Subdivision and
is a revision to the Form M dated February 6, 1996 recorded at the Essex North District Registry
of Deeds at Book 4452, Page 59.
The Grantor warrants that the aforesaid easements are free and clear of all liens and
encumbrances, that it has good title to transfer the same, and that it will defend the same against
claims of all persons.
For Grantor's title see deed from T.D.J. Development Corporation dated June 5, 1997 and
recorded in the Essex North District Registry of Deeds at Book 4770, Page 95.

RE: Bond release request
Dear Mr. Harpin:
I have received numerous requests for the bond release of$8,000,00 for work to be performed on
Salem Street and Campbell Road, by Mr. Ken Ahern, of your firm. Since all of the drainage for
Campbell Road has not been installed we cannot release this money. We will consider release of
funds one(1)year after the work is completed and paved. This assures that the trench goes
through one (1)winter.
If you need to close out the existing account we would have to generate a new bond
amount prior to the Town issuing a road opening permit for the proposed drainage and the
existing work completed. This may be necessary anyway, if the additional work raises the value of
the trench replacement & maintenance over the present $8,000.00 bond we presently hold., We
can discuss this when we meet to review your new drainage plans submitted on April 13, 2000.
Please note that the plans may require outside review.

Dear Mr.Masterson;
As agreed upon at our meeting held on November 10, 1998,with Tim Willett,Ken Grandstaff,Ken Ahern,you and
myself in attendance.The Town will inspect all utilities in the fill areas prior to their being backfilled.This is to assure the
Town that all organic material has been removed. If none are found the site will be deemed acceptable. However,if organic
material is found,all fill will be removed to allow inspection.
Also,I now understand your need to start from a manhole,other than at Campbell Road,but as agreed,the extra
manhole will be removed,prior to completing that sewer line.Remember that it is critical the sewer line must be straight and
would have to be replaced if is not straight.
