Campbell Forest subdivision is an approved residential subdivision project that has commenced construction in July 1998. The approval of the subdivision )Pas based oil each lot having an Ott-site subsurj ace disposal system. In April 1998, the applicant requested a modification to the subdivision plans to allow the construction of a Pylunicipal fpastefvater collection system instead of constructing Individual oil-site septic systenis. The North Andover Planning Board and Conseri,ation Commission approved this modification. Oil July 9, 1998, the DEP has trade a tentative determination to issue a sewer extension perinit for construction of the sewer system. The anticipated date of issuance of the permit is in mid-August 1998. The filing of an ENF is required because there is more than % mile of sefper mains proposed that are not located in the right of way of existing roadways. Description and assessment of the Project's other wastewater impacts: Mastervater flow generated in the Campbell Forest Subdivision will be conveyed through an 8" gravity server system to a proposed offsite server system, in an existing right of rvay, which will flow to a proposed pump station. Wastewater flow will then be conveyed through a force main along Campbell Road to a sewer system (proposed and designed by others) on Route 114. Approximately 21 horses along Campbell Road will be tied into the proposed server system 3. Description and analysis of alternative plans or designs for the Project or aspects thereof that would avoid or nununuze wastewater impacts: Onsite septic systems would avoid wastewater impacts to the existing server system, however, that is not a desirable alternative given soil t3 pes, and the need to eliminate potentially failing existing and proposed septic systems in this area. 4. The undersigned is submitting an Environmental Notification Form ("ENF") to the Secretary of Environmental Affairs on or before July 30, 1 998(da/e) This will initiate review of the above project pursuant to the Massachusetts Environmental Policy Act ("MEPA", M.G.L. c. 30, s.s. 61, 62 62H). The Secretary of Environmental Affairs will publish notice of the ENF in the Environmental Monitor, will receive public comments on the project for 20 days, and will then decide, within ten days,if an environmental Impact Report is needed. A site visit and consultation session on the project may also be scheduled.
