We have been in contact with each applicant seeking street acceptance information. To
date, the Planning Office has not received a completed street acceptance package.
We strontly recommend that you submit a completed street acceptance package by
Friday, April 15d'. This is to allow the Planning Board the ability to review the materials
at the Planning Board Public Hearing on April 19d'and provide recommendations to the
Board of Selectmen at its meeting on April 25'". I realize that is a long, arduous process
and we will assist you as best we can.

This memorandum has been prepared to summarize the status of the Certificate of
Compliance for Forest View Estates (DEP File #242-885). To date, the North Andover
Conservation Commission (NACC) has issued numerous Partial Certificate of
Compliances (PCOC) associated with some house lots, as well as a partial release for the
roadways (Amberville Circle; Palomino Drive; Garnet Circle; and Anvil Circle) within
the subdivision and all stormwater drainage structures.
Currently, Pulte Homes of New England has nine (9) `open' lots that are still subject to
the Order of Conditions. They are the following: Lots 38-42 Garnet Circle, Lots 22, 26, 31
Amberville Circle, and Lot 43 Anvil Circle. I have been informed by Marchionda 8s
Associates that Pulte Homes will be requesting a full Certificate of Compliance in late
May / early June.
I trust this memorandum is sufficient for your needs. Please do not hesitate to contact

Hi Leanne,
Yes, the Conservation Commission did vote to issue a PCOC for Forestview Estates for the roadways and
drainage only at their meeting last Wednesday April 13th. A full Certificate of Compliance was not requested and
cannot be granted because there are still outstanding lots to be completed and/or complied with within the
subdivision. This means that the Commission has no problem with all of the roadways at Forestview and moving
forward with roadway acceptance is no problem as far as any Conservation concerns.
I understand that Forestview has been diligent in sending this department the required affidavits upon the sale of
each house lot. However, at their meeting, the Commission was aware that 3 separate Homeowner Associations
were going to be set up for the responsibility of maintenance of the drainage systems. Because of this, the
Commission requested that an Amended EO be issued mandating that any Homeowners Association
agreement/transfer be submitted & recorded at the registry of deeds. They also requested that the O & M plan be
attached to the PCOC issued for the roadways and drainage. Please see below for further clarity.
I have notes from that meeting, but since my Associate, Pamela Merrill, was more involved with this process at
Forestview, I intended to let her continue to work on this. She is not available today and Monday is a holiday.
She will be back in the office on Tuesday. I would recommend that you speak with her if you should have any
further questions or concerns in this regard. Further, I will be getting the decision ready today to send out and
copy to the Planning Board. I will have my Associate work on the Amended Enforcememt Order immediately
upon her return. I am under a time constraint today for other obligations and am not sure if I will be available to
answer any further questions in this regard, but will make every effort to do so if further assistance is needed.
