Re: Street Acceptance and Asbuiltplan for Woodlea Road subdivision Dear Boom Members, I am in receipt of pdf files containing a' STREET ACCEPTANCE PLAN'Woodlea Road dated T/30/12 by Gateway Consultants, Inc. Lynnfield MA. and an "AS -BUILT ROADWAY PLAN' Woodlea Road dated 8/30/12 by Gateway Consultants, Inc. Lynnfield MA. I have not resolved mylar plans. My comments are as follows: STREET ACCEPTANCE PLAN 1. The title of the plan should be' Street Layout Plan'. 2. The drainage easement containing pipes structures and a basin east of cul-de-sac on lots T, 8, 9, 10, & 11 should be shown with bearings & distances 3. The sewer easement containing pipes and structures west of cul-de-sac on lots 5, 6, and cross country to the sewer pump station on Rte 114 should be shown with bearings & distances. 4. The surveyor must certify that the roadway and sidewalk, specifically the western edge of the cul-de-sac sidewalk, lies entirely within the right of way as shown on the layout plan. 5. The signature block should be Iabeled'Town of North Andover Board of Selectmen" 6. See note 2, second sentence for opinion from Town Counsel T. A new plan with revision date should be submitted. AS -BUILT ROADWAY PLAN 1. The title of the plan should be `Street As -Built Plan". 2. The Professional Engineer's certification must be atltletl to the plan (regs 5.14 last paragraph) 3. The R.P.L.S. statement must be added to the plan (regs 5.14 second paragraph) 4. The engineers statement must be atltletl to the plan (regs 5.14 second paragraph) 5. The R.P.LS. stamp must be atltletl to the plan 6. Bearings & distances must be added to the drainage easement containing pipes structures and a basin east of cul-de-sac on lots ], S. 9, 10. & 11 should be shown with T. Bearings & distances must be added to the sewer easement containing pipes and structures west of cuFdesac on lots 5, 6, and cross country to the sewer pump station on Ric 114. 8. An as built plan and profile showing the cross country section of sewer to the sewer pump station on Rio 114 must be provided. 9. An as built plan and profile showing the cross country section of drainage from DMH 2 through drainage easement referenced! in #6 above must he provided. 10Drainage from catch basin 5 must be added to profile. 11. Water services, taps, and curb stops to individual houses must be shown. Ties to red stops must he provided 12. Sewer services to individual houses must be shown. 13. Gas services, and curb stops to individual houses must he shown. Ties to curb stops must be provided. 14. Underground cable, electric and phone should be shown. 15.50'sfationing should be atltletl to roadway. 16. Bearings & distances should be added to lot lines. 17. Pavement width should W atltletl to plan. DEED AND EASEMENTS I have review a "STREET DEED' (Waodlea Road) from Michael D. Tancre8 trustee dated Dec. 23, IWO and an easement to New England Telephone & Telegraph recorded book 4797 page 241 in North Essex Registry of Deeds STREET DEED: 1. The bearings and distances sited in the tleetl match those shown on the proposed Street Layout Plan. 2. Reference's made to "certain recorded plan #12873" within the deed. Plan #128T3 is missing sheet 4 of 8. I'm not sure what legal impact this reference has to the deed. I defer to Town Counsel for an opinion on this matter. 3. Reference is made to "certain receded plan#13263" within the deed. I'm not sure what legal impact this reference has to the deed. I defer to Town Counsel for an opinion on this matter. 4. The deed references 'drainage, sewer, and water easement shown" on the above plans: A. '25' Water Main & Sanitary Sewer Easement'. There are no bearings and distances shown on this easement. This should he added to the Street Layout Plan or a plan must be provided showing bearing and distances. B. 'Drainage Easement'. There are no hearings and distances shown on this easement. This should be atltletl to the Street Layout Plan or a plan must he provitletl showing bearing and distances. C. "Drainage Easement". There are no bearings and distances shown on this easement. This should be atltletl to the Street Layout Plan or a plan must he provided showing bearing and distances. This should be added to the Street Layout Plan or a plan must be provided showing bearing and distances. EASEMENTS: 1. Reference a made to 'An easement to Massachusetts Electric Company". I defer to Town Counsel for an opinion on this matter. 2. Reference is made to 'An easement to New England Telephone & Telegraph Company. I defer to Town Counsel for an opinion on this matter.
