Pursuant to ,section .4.2 of the North Andover Wetlands Protection eg lat. .s,Arthur
Watson of A.F. Watson General Contracting, applicant, riled on behalf of the owner John Glasko,
for a Small Project Permit for work proposed at 725 Boxf rd Street, North Andover. Work involves
In-kind deck replacement at 'xl 'Nvith the installation of 5 new precast footings within. 1 0 -Foot
.duffer lone to Bordering Vegetated Wetland VW Footings shall be dug by a small excavator.
Construction access shall be located from the driveway, over the lain to the back.of the house. The
deck is and will remain approximately 88 feet from the BVW, thus outside the 50-oot No Build
Zone.

1. Prior to the start of construction the applicant shall ensure that the site contractor,has reviewed
the small project permit and is, aware of the wetland resource area and the Zits of the proposed
work.
2. Prior to cons t-ruction, erosion controls in the form,of 8" straw, wattle to be installed and staked
according to the instructions of Conservation staff
3. A re constructionsite inspection shall be scheduled-\vitli the Conservation Department and the
contractor to review erosion control installation and project Scope.
4. A post construction site inspection shall be scheduled with the Conservation Department to
verif�7 compliance and approve removal of erosion control.
5. This,permit shall expire one year ftom the date of issuance,

is allowed under this permit. The NACC has jurisdictiono-\Ter all of the wetlands and associated 100-Foot
buffer zones and 200 Bdverfront Area to the Shawsheen River on site and any work that involves
