AGREEMENT WITH THE TOWN OF NORTH ANDOVER
THIS AGREEMENT made and entered into this3ati day of November, 2025, by and between United
Teen Equality Center (UTEQ, a nonprofit corporation located at 35 Warrant Street, Lowell,
Massachusetts 01852, hereinafter called "VENDOR", and the and the Town of North Andover,
("TOWN") a municipal corporation duly established by law and located at 120 Main Street, North
Andover, Essex County, Commonwealth of Massachusetts.
The above parties agree that the procurement of Mattress Recycling Services for the Town from
VENDOR will be governed exclusively by the "COMMONWEALTH OF MASSACHUSETTS —
STANDARD CONTRACT FORM" and it is incorporated by reference into this Agreement, along
with its terms and conditions and all terms contained in FAC90 Category 2.The Commonwealth's
contract and Terms & Conditions shall take precedence over all other language.
This Agreement will commence upon execution and expire on October 31,2028.The pricing shall
be $40/ per unit until October 31, 2026, a 5%OPTIONAL increase at contractor discretion for the
time period of 11/1/2026-10/31/2027 and another 5% OPTIONAL increase at contractor
discretion for the time period of 11/1/2027-10/31/2028. Pricing for drop offs from the
Department of Public Works will be$20 per unit.
Multi-year Agreements are subject to annual appropriation.
Signatory for the Vendor certifies that they are duly authorized to execute this Agreement on
behalf of the Vendor, and that affix its corporate seal hereto making this Agreement and all of its
terms valid and binding on the Vendor.
