To the best of my knowledge,information, and belief the work proceeded in accordance with the requirements of 780 CMR and the design documents approved as part of the building permit and that I or my designee: 1. Have reviewed,for conformance to this code and the design concept,shop drawings,samples and other submittals by the contractor in accordance with the requirements of the construction documents. 2. Have performed the duties for registered design professionals in 780 CMR Chapter 17, as applicable. 3. Have been present at intervals appropriate to the stage of construction to become generally familiar with the progress and quality of the work and to determine if the work was performed in a manner consistent with the construction documents and this code WITH THE EXCPETIONS AS NOTED BELOW IN#4. 4. This is issued at the request of the town for use for a TEMPORARY CERTIFICATE OF OCCUPANCY. The site is not fully complete and additional site work is required. As-Built Survey: An as-built survey stamped by u registered land surveyor is required as an approval condition of the town and project specifications. An Interim As-Built dated June 6Lh received following the site inspection is incomplete and does not comply with the requirements. DZ| is not able to determine acceptability for afina| certificate of occupancy or nignof[ With respect the As-Built Survey, many items are missing such as finish grades and contours, detention basin contoured to evaluate volumes, all parking mtr|p|nX, zoning umtbuc|m, ui8noge' vvu||o' fencing, surveyors xtunnp and other pertinent information. Until review of 0nu| as-built survey. DZ| cannot provide u 0nu| affidavit without these items listed to certify full compliance to the approved documents. 2. Pavement: Inconsistent surface coot. Drive aisles are acceptable. Parking spaces visually show o large amount of aggregate as if they did not receive the surface coat or rolling. This could be problematic during plowing. 3. Topsoil: Finish grade has not been established throughout the site, Landscape areas should be flush with the edges of patios, vvu||wwuyo and curbs. Stones and debris are visible through out the site. The specified amount of topsoil may have been provided' however since finish grades were not established the amounts and locations vary. Landscaping: The number of plants correspond to the plan. However, the Honey locust trees are undersized. Many of the plants were not spaced properly and are not planted in straight or offset lines leading to a messy look. Many of the trees seem to be mounded and perhaps planted too deep. GC to confirm trees were planted with the flare at finish grade. Undersized plants to be addressed with respect to credit due. One year warranty will not be initiated until landscape signoff and at the time of the punch list this is not possible. 5. Lawn: The lawn is not acceptable; loam depths vary and the entire lawn is full of weeds. The lawn and all weeds should be removed. The entire site needs to be finish graded. Loam and seed Submittals need to be submitted for approval and the site needs to be re- seeded within the dates specified in the specification. Maintenance then needs to be provided as outlined in the specification. 6. Lighting: Acceptable- as observed during the day. Light pole base at the service area set with 20-inch reveal from grade and not 24-inches for full bumper protection. 7. Rear Patio: There is a large unstable "bubble of pavers" that lift up when walking on just as you enter the patio adjacent to the rain garden. This area has been incorrectly installed, presents a safety/tripping hazard and does not meet the standards outlined in the specification or by the MAAB. There is also a transition differential lip at the wall', and patio area at the east facing building door/walk. 8. Rain Gardens Seeding: Side slope seeding is outstanding, the stone on the bottom of the swales is not riverstone and does not match the detail. GC to provide a submittal for seeding of the stormwater/raingarden basins, a submittal for the riverstone. Loam and seed submittals need to be submitted for approval and the site needs to be re-seeded within the dates specified in the specification. 9. Drainage Swale: The required drainage swale along the southern abutter fence has not been installed per plan.This Swale is required to control and alleviate runoff from the site and off-site areas from entering the abutter properties. DZI was informed that alternate solutions are in process to address this swale deficiency outside of the DZI design. Item outstanding. 10. Domestic and Fire Protection Water Service:Water line was designed from Sutton Street but appears to be connected to Surrey Street. Connections, size,water gates and process for changing unknown and this is to be addressed by the town and water department for conformance to their requirements. 11. Fence: The wood fences along the southern abutter and utility area at building do not meet the plan detail or specifications (material, hardware, nails, staining etc.). Not acceptable. 12. Signage and markings: The turn arrow as the drive enters onto Surry Street has not been painted on pavement. Page 4 of 9 6/8/2023 North Andover Adult Center June 6, 2023 Page 3 13. Sanitary Sewer: DZI informed that final connection to the street has not been performed and that the connection is into existing clay pipe. 14. Drain Manholes:All drain manholes need to have concrete shelf poured within for smooth invert transition. The ones opened did not have this installed within. 15. Drainage Area: The sidewalk designed at the southeast corner of the building has been extended past the parking area to the abutting housing parking lot and drive area. As a result, the stormwater from the abutting pavement runs directly over the sidewalk's curb opening and into the eastern raingarden. This situation results in erosion and impacts the detention basin effectiveness which was not designed to accept off-site parking areas. 16. Layout and Project Changes: Note that aeras in the main parking area and perimeter walls/grading were redesigned following the bid drawings. These changes were designed by others and reviewed by the town separately. The site observations within address the plans referenced within this observation report only. 17. Please notify us of any misunderstandings or required interpretations within five days, as we will proceed on the basis that these items are agreed to by all parties.
