Basic Information

SECTION 1: CON STRUtTION ACTIVITIES
1. Contact the Owner in writing at least seven (7), days, prior to the start of construction.
2. Place the site sign (with CEP, assigned project number and contact numbers) prior to
any work on site.
3. Install the erosion control KMPs as shown on the construction documents.
4. The silt fence and silt sock line shall bleinspected on a weekl usis; any breaks in the,
lire shall be repaired as,soon as possible.
5. All erosion and sedimentation controls sha�ll be inn acclordance, with the DEP'ls Erosion
and Sedimentation Control Guidelines and the USDA SCS Erosion and Sedimentation
Control during site, development.
6. All temporary stockpile areas shall be protected by silt socks, and shall be covered
with a tarp to prevent moisture intrusion and dust concerns.
7. All areas within the, limits of work no receiving a surface treatment and any disturbed
areas outside of the firnit of ork shall be stabilized with mulch or,seed immediately
upon completion of construction activity. In no case, shall an area be left unstabilized
for more than 14,days after the, construction activity yin that area has ceased.
8. All erosion control measures shall be inspected after any, rainfall of 0.5" or greater.
9. All catch basins downstream lof,works areas shall be ringed with silt socks and covered
with a sediment filter,until' all up-gradlent disturbed, areas are stabilized,
1,0. Any outlet orifices, shall be ringed with silt socks until the, detention structure or,

infiltration area is stabilized, if applicable,
11. All slopes greater than 3:1 shaill be stabilized with an erosion control blanket.
12. The contractor shall keep additillona] silt fence and straw bales on site to be able to
respond immediately to any emergency conclition.
13. All, proposed drainage structures (catch basins, manholes, outlet control structures
and detention systems) should be clewed at the end of construction and at any time
the sediment within the, structures, equals, 12" deep.,
14. The contractor shall confine activities to, the limit of wor'l. and only disturb the,
minimum area, necessary.
15. All illicit dis,charges are prohibited.
16. The entire project arlea shall be, stabilized with vegetation upon completion of
constructlion and prior to the removal of the erosion control devices.
17. The Order ofConditions,issued by the North Andover Conservation commission shall be
incur orate 'in the clonstruction contract documents.

SECTION I: POST-DEVELOPMENT ACTIVITIES
,PART A- GENERAL
• It shall be the responsibility of the owner to implement the procedures outlined in
this pian.
• The closed drainage systern shall be inspected every 6 months, and any excess
sediment within the structures or detention systems, shall be properly disposed of.
• Any problems found with the drainage system shall be repaireld within one week of
d iscovery.
• The Owner shall employ a qualified professional to, perform periodic maintenance,
as described in this plan.
• All maintenance personnel shall be trained annually on the operation, and
maintenance procedures. A training log shall be maintained for the erec,orld to,
,document the annual training of employees.
• Inspection logs are, included with, this O&M Plan. The qualified professional shall
p ide the Owner with maintenance lags,after each inspection or corrective action.

provide copmels to the DEP and Conservation Commission ifrequired.

PART B - BMP MANAGEMENT
Each Best Management Practice shall be maintained per the below requirements:
SYNTHETIC TURF FIELD— Est. Annual Cost- $8,000
0 Perform regular grooming of the synthetic turf to redistribute inf III as recommended
by the turf manufacturer, typically every two to four weeks cluing active, seasons.
0 Perform regular trash/litter cleanup as needed.
Perform visual inspection of turf seams twice a year,.
Inspect cleanouts, and drain manholes after every maj r storm during�, the first 3
months, of operation and twice a, year thereaf,ter.,
CATCH BASINS/AREA DRAINS,—EsIt. Annual Cost 1-$15,tOOO
• Inspect and clean at least four times, per year (quarterly),.
• Sediment shall be removed when the depth, is greater than one, half the distance
from the bottom Invert to the manhole f'loor'.
• Use, of a vacuum truck Is the preferred cleaning method.

Prevent or reduce pollutant runoff performing periodic landscape,

STORING MAT ERIALS AND WASTE PRODUCTS
All materials als stored on site shall be stored in a meat and orderly, fashion, in their
appropriate containers, and under a roof or offer secure enclosure. Waste
products should be placed in secure receptacles until they are emptied

MAINTENANCE OF GRASS AREAS
The-Owner will be responsible for these activities., Maintenance typically involves

fertilizer.. Grass, cuttings are typically left in phase for nutrient retention., Wind
blown leaves, are, removed until late fall.

PROPER MANAGE MiEN'T OF DEICING CHEMICALS AND SNOW
Snow disposial shall be in accordance with the Department of Environmentai
Protection, Bureau of Resource Protection,, Snow, Disposial Guidelines, Guidelline
No. BRPG01-101. In general, snow w'11,11 be plowed in accordance with standard!
operating procedures. Whenever possible, the use of environim gent ally friendly
alternatives (e.gl., calcium chlorlde, and sand instead' of salt for melting ice) will be

Snow on athletic fields and tennis courts shall not, be plowed on a regular basis.
No sand, salt, or chemicals shall be spread on athletic fields or tennis courlts,.
Natural seasonal snow melting will be the preferred method of snow removal.Any
