Dear Members of the Planning Board:
On behalf of Davita Healthcare Partners, Inc., Civil Design Group, LLC is pleased to submit the enclosed
Application for a modification to the 03/04/1997 Site Plan Review — Special Permit Decision pursuant to
Special Condition ##1 which states that `There shall he no more than 19 dialysis stations at this site at this
time. if the applicant wishes to create additional stations, he must appear before the Planning Board to
request a modification to this decision. ' Earlier this year, three (3) dialysis stations were installed and upon
further file research by the Town, the above referenced condition was discovered and as a result, the
Planning Board issued a Temporary Approval of Modification on 08/20/2015, thereby providing the
Applicant three (3) months to file a formal modification to the 1997 Site Plan Review — Special Pen-nit
Decision. Accordingly, we have enclosed the following information:
• Signed Special Permit—Site Plan Review Application
• Certified Abutters List
• Site and Landscape Plan, dated 10/07/2015
• Plant List Documentation
• Davita Parking Letter, dated 10/05/2015
• Check No. 1269 in the amount of$200 (application fee)
Since the improvements for this site are internal to the building and limited to the addition of three (3)
dialysis stations, it is our opinion that certain review criteria/design guidelines items of Section 8.3.6 - Site
Plan Review are not applicable. However,the applicant is proposing the following exterior improvements at
the request of Planning Staff:
• Restriping the 37 onsite parking spaces to clearly delineate parking areas for patients, employees and
delivery vehicles as depicted on the Site and Landscape Plan.
• Installing a new 15'x 10' dumpster pad with 6' high stockade fence enclosure as depicted on the Site
and Landscape Plan.
• Installing a new monument sign in the existing location as depicted on the Site and Landscape Plan.
• Installing 37 shrubs, perennials and groundcover as depicted on the Site and Landscape Plan.
