PROJECT
On November 17,2015 the Planning Board voted to amend the previously approved Site Plan Review
Special Permit Decision, dated March 4, 1997,to modify"Special Condition 1",to allow for in increase
from a maximum of 19 dialysis stations to no more than 22 dialysis stations. The site is located Within the
Industrial S Zoning District(I-S).The March 41'decision was to allow a new use in an existing structure
which contains new processes not normally associated with the existing use.
On April 18, 2017 the Applicant requested to amend the approved plan and alter the handicap parking
space with the crass-hatched space adjacent to the proposed location towards the entrance.Additionally,
the Applicant has enlarged the handicap parking space at the front of the building to be Van Accessible
now measuring 10' and the crosshatch measuring 8' from the original 9' and 9'.The Applicant also
added accessible parking signage.

FINDINGS
The proposed changes are deemed to be Insubstantial and do not alter either the intent or the conditions of
the original Site Plan Review Special Permit.
All plan revisions will be reflected on the final as-built drawings.
