Conservation Commission Members;
On, behalf of the owners,,Kevin+Alison Sparks,of 325 john Street, I am requesting a Certificate of Compliance tom
the Order of Conditions,DEP File#:242-18I 16. The owners had also received a Watershed Special Permit from the
Planning Board,and the Planning Department is copied on this letter to alert them to the project completion.
The Order of Conditions was 'issued to,the Spam-s for,this project and they are still the current landowners. They have
read and understand the recorded,Order of Conditions. The Sparks had received an extension until 11-30-2025.
Additionally,the Watershed Special Permit had been extended by the Planning Board expiring on August 17,2024.
The Clertificate of Compliance is to be granted to Kevin+Alison Sparks

of,As part this request,,I am attaching a completed WPA Form 8A application, a check for,$200.00, and 2 (two)full
size As-Buflt plans.
I personally Inspected the site on August 9,2026 and found the,bite to be fully,stabilized by means,of grass and other
landscaped,materials. The detached garage is fully constructed and the crushed stone driveway transition to the existi�ng
gravel drivieway i's complete. The concrete,dell was installed per the approved plan and,the down,spout/roof drains
are shown,on the As-built plan.'Thle wetland emblems were installed by the owner on 4"A" ressure treated posts
and/or,trees— 11 total,(per Condition 415 1). 1 have included,fivie(5)site photos from the 8/9/26 site inspection to
accompany this CO,C request.
The site was constructed in substantial compliance with the Order of Conditions. Additionally,the detached garage,gas
and sewer services, crushed,stone driveway extension,and site grading was completed in accordance with the a I proved
design plan for both the Conservation Commission&Pla nning Board approvals.
