In accordance with the Fire Department ' s letter dated June 28 , 1982 , fire hydrant distance between Lot 18 and Lot 1 shall be reduced by having all hydrants on develop- ment no more than 500 feet apart. All hydrants shall have one 42 inch outlet and two 22 inch outlets . 2 . In accordance with the Board of Public Works ' recommen- dations dated July 1 , 1982 - a ) An 8 inch ductile iron cement lined water pipe, seal coated inside , tar coated outside , shall be installed 15 feet from the property line on Kara Drive 5 feet below finished grade for 2 ,652 feet. b) Eddy breakflange hydrants shall be installed to town specifications at Stations 6+90 , 11+509 15+85 , and 19+80 . These hydrants shall be installed with 6 inch Mueller or equal gate valves (open left) with sliding type cast iron gate boxes marked "WATER" according to AWWA specifications L500 , 52T . Eight inch Mueller or equal gate valves (open left) shall be installed at Stations 0+0 , 11+40 , 19+70 , and 25+60 . c) All conditions set forth in 2 (b) shall pass leakage and pressure test in accordance with Section 13 AWWA Standard C 600 and shall be chlorinated according to C601 Standard procedure for disinfecting water mains . d) Adequate public liability , property damage , insurance , and street opening bond shallberequired 'for work on both Lisa Lane and Johnson Street in the amount satisfactory to the Highway Surveyor. e ) The 315 feet of 8 inch V. C . sewer pipe installed along the centerline of Johnson Street and 1 , 174 feet through the Giard property and ^rive shall have five (5 ) inch vetrified clay stubs installed to each property lin-e of existing homes , f) The road or sewer grade on Kara Drive from Station 15+00 through 16+0 shall be modified to allow a 4 foot minimum cover over the sewer . g) All precast manholes shall conform to ASTM Designation C4-78 and meet the followings 1 ) wall thickness shall not be less than five inches , sections shall have tongue and groove joints with an approved round or fin type gasket . In accordance with the Highway Surveyor's letter dated July 6, 1982 : a ) Additional drainage shall be required due to the exis - tence of 6% grades . b ) Two catch basins shall be installed at the intersection of Lisa Lane and Kara Drive . Kara Drive will require the installation of two catch basins at Stations 25+00 and 25+50. c) Sub-drainage in shoulders shall be required with possi - ble stoning of joints in pipes . d) Headwall plan specifications shall be reviewed at the Highway Department . e ) In order to meet slope and shoulder requirements , slope easements will be required where necessary . f) All easements shall be fully dimensioned on the defini - tive plan and submitted in a recordable form deeded to the Town of North Andover. g) Any drainage from cellar or lot development shall be tied into the proposed drainage system with appropriate plans to be submitted and subject to Highway Department approval . h ) Sub-drainage and other drainage problems resulting from the construction phase will be determined in the field by the Highway Surveyor and will be incorporated in Y s Pp drainage plans b applicant . P i ) No drainage structures shall be allowed on filled land unlessp rop er fill material is used to eliminate settle- ment problems . j ) Cape Cod berm will be required where and when necessary to be determined by the Highway Surveyor. In berm areas road pavement and gravel width shall be increased by three feet . RECC�yED Order of Conditions DtStEL s,,ZNG "Mills Hill Estates " Preliminary Plan of Land TOWNcIF1111 ER November 17 , 1982 NpRTH a Page 3 17 W1916 k ) Leveling areas to be shown as intersections where grades exceed requirements and be detailed on the profiles . 1 ) A siltation and erosion control plan shall be sub- mitted with appropriate conditions under the approval of the Planning Board and Performance Bond shall be retained to ensure the conditions are met . m) All masonry shall be of solid bed and end joints of a good quality cememt mortar. Clay bricks are to be used where necessary . 4. In accordance with the Conservation Commission ' s letter dated July 1 , 1982: a ) According to Chapter 131 , Section 40 of the Mass. General Laws , and the North Andover Wetland Protection By Law , a Notice of Intent shall be submitted if any construction takes place in or within 50 feet of known wetlands . 5 . The definitive plan shall show all existing and proposed contours in accordance with Regulation III , B ,3 ,K. 6 . All utilities for the subdivision shall be shown together on plan and profile sheets . 7. Kara Drive at Lisa Lane shall be crowned at Station 0+0 so as not to allow surface water from Lisa Lane to flow down Kara Drive. 8. The catch basin on Lisa Lane shall be incorporated into the Kara Drive drainage system. 9. A set of catch basins will be required at Station 1+50±. 10. All utilities for the subdivision will be designed to fall within the paved portion of the street . 11 . Sub-drainage will be constructed in the shoulder areas for all cuts greater than 6 feet . 12 . A 6 inch cape cod berm will be required on all street slopes of 4% or greater. 13. Proposed dwellings with cellar floor elevations will be shown for all lots and sewer services and inverts will be shown for lots 10 , 11 , 12 , 14 , 18, 23 , 22 , 15 , 21 , 16 , and 17 . 14'. " 15 . Complete topographic details and existing utilities will be shown for fifty feet (50 ' ) each side of the centerline of any and all utilities through Giard property and driveway to Johnson Street and Johnson Street and any other drainage flows , accesses , for the limit of all construction . lb . All verticle curves shall be 100 feet ( 100 ' ) in length minimum . 17 . A detailed topographic plan shall be submitted which clearly shows the watershed boundaries and destination of surface water from this locus. Drainage shall not change watersheds . 18. Plan size shall be 36" x 24" for the definitive plan . 19. The entire width,- idth between exterior street lines shalhe graded to pitch shoulders toward the gutter line. Stabilized shoulders and grass plots shall be constructed on each side of the roadway with exterior street line grades 8 inches above gutter grade. 20 Definitive subdivision plans and profiles shall conform with all other aspects of the Rules and Regulations . 21 . Any lot not services by the Sanitary sewer system will be designated on the definitive plan to be recorded as "Not A Buildable Lot without Prior Board of Health and Planning Board Approval . 22 . Require the plans to mitigate any increase rate of run off to zero . Provide compensating storage to account for any and all filling such as the increased rate of run off from development. 23 . No drainage pipe larger than a 15 inch shall be allowed to exist from a catch basin . 24 . All off-site utilities will be designed and submitted with the definitive plans . 25 . Require a Statement of Environmental Impact with empahsis on : 5a (1 ) ; 5a (3) ; 5a (4 ) ; 5c(1 ) ; 5c (2 ) , 5c (3 ) ; and 5e (7) . 26. All off-site proposed work shall be documented with written authorization from land owners or public authorities having jurisdiction . 27. In the event maintenance is required on the Giard property sewer easement , the town shall not 'incur the responsibility and/or cost of landscaping or pavement repair . 29 . The definitive plan shall address the Rea Street alternative drainage through the two existing culverts . The plans and drainage calculations shall detail the culverts , brooks , and associated rate of runoff. All upgrading costs shall be incurred by the applicant , and supply the necessary easements deeded to the town of North Andover for maintenance purposes . 30 . No piped drainage from the proposed subdivision shall be directed toward Johnson Street .
