The North Andover Planning Board has voted to APPROVE said plan, subject to the
following conditions:
1. That the record owners of the subject land forthwith execute and record
a "covenant running with the land", or otherwise provide security for the con—
struction of ways and the installation of municipal services within said sub—
division, all as provided by G.L. c. 41., S. 81—U&

2. That all such construction and installations shall in all respects
conform to the governing rules and regulations of this Board.
3. That, as required by the North Andover Board of Health in its report to

d. Final as-built plans shall,be submitted. to the Planning Board prior to final release
of security.
e. The applicant lownerldeveloper shall be aware of the fact that approval of this
subdivision under the Planning Board's Rules & Regulations will not relieve said parties
of the responsibility to other property owners upon whose land may be affected by indirect
or direct.development storm water discharge.

5 . In accordance with the hoard of Public- Workh recommendations:
a ) An eight (8) inch ductile iron cement lined water pipe, seal
coated inside, tar coated outside shall be installed on Anne
Lane. This is for 450 feet of eight inch pipe installed
fifteen feet from property line with five feet of cover
below the finish grade of the street.
b) An eddy breakflange hydrant to North Andover specifications
will be required at Sta. 4+40. Said hydrant shall be instal-
led with a six inch Mueller or equal gate valve (open left)
complying with AWWA specifications L500 52T with sliding
type cast iron gate box marked "Water. "
c) A twelve by eight inch tapping sleeve and gate will be
required at Salem Street.
d ) All of the above work must pass the leakage and pressure
tests performed in accordance with Section 13 of AWWA
C600 and shall be chlorinated in accordance with C601
Standard Procedure for Disinfecting Water Mains . Adequate
public liability and property drainage insurance as well
as a street opening bond in an amount satisfactory to the
Highway Surveyor must be provided for the work on Salem
Street.
6 . In accordance with the Fire Department' s recommendations:-'
a) The proposed name , Anne Lane, should be changed to eliminate
any confusion. The proposed street name shall be submitted
pBroaiorrd.to signing the definitive. and approved by the Planning
b) All lot numbers shall be posted during construction.
c ) Permanent house numbers shall be posted prior to occupancy.
d) The roadway should be made suitable for emergency vehicle
access prior to occupancy of any residence.
e) No open barning is permitted except as allowed by the Fire
Department.
7 . A Notice of Intent shall be filed with the *forth Andover Con-
servation Commission as required under Chapter 1319 Wetlands
Protection Act and 'local Conservation Commission' s regulations.
Any changes required may be cause for reconsideration of the
subdivision under M.,3. L. , Ch. 41.
8 . All requirements of Subdivision Control Laws pertaining to
' Street Trees ' shall be enforced by the Tree Department.
9 . In accordance with the Highway Department' s recommendations :

a) Stone bounds shall be 8" square by 4 ' long.
b) Grade on shoulder area to be 1/4" per foot.
c) All utilities to be buried a minimum of 30" deep and
have a safety ribbon laid in trench.
d) Headwall to be built to the Highway Department' s speci-
fications available at the Highway Department.
e) Twenty foot wide drainage easement shall run continuous
to abutting property.
f) Road paved width should be twenty two feet with 18"
overlap of gravel .
g) Road crown shall be 3/8" per foot.
h) Two catch basins shall be situated at the intersection
of Salem Street and the proposed road.
i ) Cul-de-sac shall be centered on the roadway and not off
set as shown. Cul-de-sac to be 100 ' paved circle.
10 . A stump pit burial plan shall be submitted to the Planning
Board and Highway Department for review and approved prior
to any on site construction.
11. A covenant restriction shall be placed on the deeds of Lot 1
and Lot 2 describing the narrow strip of land along both sides
of the proposed road as no access ' buffer strips ' . Said
covenant will hot permit any further lot development off of
the proposed roadway.
12 . The Developer shall be required to record all plans and docu-
ments signed by the Board in connection with this Subdivision
in the North Essex Registry of Deeds. The Developer shall
be further required to provide the Board with a Certified
copy of all such recorded plans and documents. Should the
Developer fail to comply with any part of this provision,
the Board reserves the right to retain an attorney to do so
at the Developer' s expense.
