CARLETON FARMS WEST CONDITIONS: March 20, 1978
1. No work shall be done in the subdivision until security in the form of a bond or
covenant has been duly recorded and the plans are- signed and recorded;
2. that all street trees shall be in accordance with the requirements of the Tree Warden;
3. that adequate public liability and property damage insurance as well as a street
opening bond be provided in an amount set by the Highway Surveyor and Board of Public
Works;
4. that all construction improvements shall be built in accordance with the "Rules do
Regulations Governing the Subdivision of Land, Town of North Andover, Ma, 1973";
5. that all construction improvements pertaining to this subdivision be completed in
full by June 1 , 1980;
6. surface drainage or run-off on any lot shall not be directed to neighboring properties
to cause adverse conditions;
7. 15 ft. easement as on previous recorded plans be deeded to the Town;
8. 8 in. water main per letter of 11-30-77 from Desun Corp. as agreed to by Board of
Public Works;
9. Fire hydrants per specifications of the Fire Chief; one additional fire hydrant;
10. 40 ft. buffer restricted to "no work" on west portions of Lots 4 & 5;
11. 2 ft. contour plans of the 40 ft. buffer zone to be submitted to the Planning Board;
12. any modifications by the Conservation Commission to the subdivision plans shall be
submitted to the Planning Board;
13. percolation tests to be made at site of leaching fields prior to the start of any
road construction;
14• no lot shall be built upon until a subsurface sewage disposal system permit is
granted by the Board of Health;

Plan dated November 8, 1977
Dear Xr. Lyons:
At Planning Board meeting of November 28, 1977 it was unanimously voted
to reconsider the original vote to endorse the above—named plan not requiring
approval under subdivision control law.
Upon motion duly made and seconded, the Board voted unanimously to disap—
prove endorsement of the subject plan because a mutually agreed upon request
between the Planning Board and the applicant's representative, Mr. Gelinas, was
not honored. The plan was not amended to add dimensions to an existing frame
building and was picked up after 14 days for recording without the requested
changes.
