A Notice of Intent shall. be filed with the Conservatio,D Commission as required under Chapter 131 - Wetlands Protection Act and local wetland regulations . 2 . Any drainage from cellars or lot development shall be tied into designed drainage systems to be shown on the definitive plan, and approved by the Planning Board and Highway Surveyor. Drainage not shown on the definitive plans will not be added during con- struction unless required by the Highway Surveyor. 3 . No provisions have been made for future access or connection to abutting land. The applicant has not taken any stand to extend the cul-de-sac to the property line. 3. The applicant has not addressed the need for a 'parcel' of land within the subdivision as open space for neighborhood use. (See Preliminary Plan decision - Condition #13 - dated July 29, 1986. 4. The applicant has not complied with the State DPW requirements for access/egress to the subdivision (Condition #12 of preliminary decision. ) 5. No pedestrian access to land owned by the Commonwealth of Massachusetts has been shown on plan. No attempt has been made to provide same. 6. No proposed grading for drainage; and existing and proposed contours are unclear as to what is being filled and what is being excavated. The Planning Board is of the opinion that the amount of tree removal and foliage line is not accurately represented on the definitive subdivision plans . 7 . Areas to be used as a stump pit have not been delineated on the definitive plans for review and approval by the Planning Board as required in condition #5 of the preliminary decision. 8. Lot #5 is considered an unbuildable lot as noted in the Board of Health' s letter dated May 27, 1986. 9. It is unclear at this time that the subdivision will have sufficient water and pressure for adequate fire protection. Additional information and evidence is needed to determine the year round functioning capacity of this system for total dependence for fire protection. 10. These tests consisted of two percolation tests ; one three minutes per inch, and the other five minutes per inch resulted. Additionally, a confirming test pit (deep observation hole) was performed as requested by North Andover Health Agent, Mike Graff. This test confirmed the presence of the required four feet of parent soil material , with no water observed to a depth of 6. 75 feet below grade. All tests were witnessed by Mr. Graff. Previously, a sucessful test pit (lot 7, test pit # 1 ) had been performed on May 14 , 1985 . Based upon the excellent percolation test results (I feel five minutes per inch is in the optimal range for successful effluent renovation) and confirmation of the requisite four feet of parent soil material demonstrated by both the earlier and most recent tests, in my professional opinion there is every reason to believe that Lot 5 will support a subsurface sewage disposal system design. I would therefore , be pleased to proceed with the design at your earliest convenience. Enclosed please find a preliminary sketch of a proposed system design depicting a 1500 gallon septic tank, distribution box, and two leaching pits (with appropriate reserve area) for your examination. See Subdivision Plans Sheet One SECTION G ENVIRONMENTAL REPORT Environmental Report This Statement of Environmental Impact is presented by first giving the specific wording of the subsection as given in Part III-B-5 of th*e Rules and Regulations followed by the applicant 's response to that specific request for information. PA-rt III b 5_ The statement of Environmental Impact should include the following: a) A set of plans at uniform scale shall be submitted in conjunction with the Environmental Impact Statement , . encompassing the entire subdivision on a single sheet riot larger- that 42" by 60" showing the following: (1 ) Present and proposed topography at two ( ) foot intervals, with graphic_ drainage analysis, location of existing structures, including fences and wails; See design sheets two and three. (2) Vegetative cover analysis, including identification of general cover type, (wooded , cropland brush , wetland, etc. ) , location of all major tree- groupings, plus other outstanding trees or other botanical features, important wildlife habitats, and identification of areas not to be disturbed by construction; See design sheets two and four, Wooded area consisting of hardwood and softwood secondary trees . .Wetland area in the South sector. Wooded areas- to be left intact where possible.. <: (_3) Approximate surface and subsurface water level and seasonal high ground water maria; 78.00' (4) Soil types based un U.S.D. See Section D See:.also. :Design Sheet Four. � . b) Physical Environment ( 1) Describe the general physical conditions of the site, including amounts and varieties of vegetation , general topography, unusual geologic, scenic, and historical features, trails and open space links and indigenous wildlife; The site typifies the topography and vegetation of this area of New England. The front portion, bordering Turnpike Street, is forested by secondary softwood and hardwood growth, with varying slope conditions , and intermittent outcroppings of stone. The wetland area is protected by dense underbrush. The remainder of the site is flatter, but with similar tree group- ings. The perimeter of the site is outlined by stone walls . The southernmost point of the site is protected by a large expanse of ledge. (This is well outside of any construction area. ) ) Describe how the project will effect these features, The land's contours will be reshaped where houses will be built, and driveways constructed. Regrading will be necessary for the construction of the proposed roadway. In general, natural landscaping will be emphasized for the houselots. No filling of wetlands is proposed. Except for construction areasi (house, driveway, septic system, utilities and roadway) . no trees will be removed. A fifty foot wide "Green Belt" is proposed for the Turnpike Street side of the project. 3) Provide a complete physical description of" the project, and relationship to surrounding area. Eight house lots and approximately 700 feet of roadway will be constructed. Drain lines and underground electric service will be laid. hots range in size from 1 to 3W acres . Proposed house locations and lot sizes will result in the site's wetlands being undisturbed. The project is consistent _with the area 's prevailing land use of residential housing. c) Surface, Water, and Soils (i ) Describe the location, extent , and type of existing water and wetlands, including existing surface drainage characteristics both within and adjacent to the project; s P � An isolated wetland area is contained in the site . This wetland area abuts lots 3, 5, and 7. i (2) Describe the methods to be used during construction to control erosion and sedimentation; eg. , use of sediment basins and type of mulching, matting, or temporary vegetation, describe approximate size and location of land to be cleared at any given time and length of time and exposure; covering of soil stockpikes; and other control methods used. Evalaute the effectiveness of proposed methods on the site and on the surrounding areas, Erosion and sedimentation will be controlled by the use of fabric fencing$ sedimentation areas, and hay bales. Sedimentation areas will be constructed of stone and hay bales . The location of fabric fencing is shown on sheet four of the plans. Two rows of staked hay bales will be placed around each completed catch basin. 3) Describe the permanent methods to be used to control erosion and sedimentation. Include description of a. any areas subject to flooding or ponding, b. proposed surface drainage system, C. proposed land grading and permanent vegetative cover , d. methods to be used to protect existing vegetation, e. the relationship of the development to'-,the topography, •f . any proposed alterations of shore lines, marshes or seasonal wet areas, g. any existing or proposed flood control or wetland easements, h. estimated increase of peak run-off caused by altered surface conditions, _ and methods used to mitigate any impact and/or return water to the soil . Erosion and sedimentation will be controlled on a permanent basis by the re-vegetation of all areas disturbed by i construction. Per Planning Board Regulations, the road shoulder will be loamed and seeded. In areas of homes , lawns will be planted. Also see Sections E and I. 4) Completely describe sewage disposal methods. Evaluate impact of disposal methods on surface water, soils, and vegetation; On site sewage disposal systems will be used. Impact on surface water, soils , and vegetation will be minimal because of proposed lot sizes, and strict adherence to North Andover and State Sanitary Godes . d) Subsurface Conditions. (1 ) Describe any limitations on proposed project caused by subsurface soil and water conditions, and methods to be used to overcome them; Based on the results of extensive sub-surface exploration, it is not anticipated that there will be any limitations to the project because of adverse soil or water conditions. (2) Describe procedures and findings of percolation tests conducted on the site; See Section D (: ) Evaluate impact of sewage disposal methods on quality of subsurface water; Minimal impact as• per #4) above e) Town Services (1 ) Describe estimated traffic flow at peak_ periods ,and proposed circulation pattern; The traffic flow per day is estimated to be 96 vehicles. The (leak flow would be 38 vehicles_.per hour. The project has complied with the State DPW requirement for access/egress into the proposed subdivision. See Sheet Two for proposed acceleration/deceleration lanes. (2) Describe locations and number of vehicles accommodated in parking areas; Each home will have a driveway and most will have a garage. Each home can accommodate at least 3 vehicles. ( ) Describe effect of project on police and fire Protection services; The construction of new roads and homes will increase the work of the Police Department. and Fire Department. (4) Describe effect of project on Public Works Department; The - project will increase the work of the Public Works Department. (5) Describe effect of project on educational services; The project will eventually increase the number of school age children in North Andover. (6) Describe effect of project on the town water supply and distribution system; No effect; on-site deep well systems. (7) Describe effect of project on the town sewer system. No effect; on-site systems f) General Impact ( 1) Summarize briefly environmental impact:'on entire town with supporting reasons. . • The Subdivision will have a negligible impact on the Towns services because of its small scale and increased tax revenues. Increased demand for public services will be offset by the tax revenues generated from the $2,000,000 in projected assessed values. The project's environmental impacts are also negligible. No filling of wetlands are proposed. In addition, a-one to one cut-fill ratio •is made possible for proposed roadway by virtue of existing topography. See road profile, Sheet three. Erosion and sedimentation will be controlled in the following manner: 1 . Siltation control ' f encing#will be placed at the base of all cut and, fill slopes that could cause siltation of surface waters and along the roadway as shown on the site grading sheet. 2. Siltation basins will be constructed at the discharge end of all drain pipes and kept in place until the roadway is paved and the grass has grown on the shoulders. a. Hay bales will be placed around each. catch basin inlet. 4. , Road shoulder will be loamed , seeded and mulched. # As per North Andover Conservation Commission Order of Conditions issued for this project, hay bales will be utilized in con- junction with the described siltation control fencing for wetland areas.
