In accordance with the Fire Department' s letter dated August 10 , 1983,. a fire hydrant shall be placed at the end of the cul -de-sac in front of Lot No . 49 . 4. According to the Board of Public Works letter dated August 29 , 1983: a ) The Eddy breakflange hydrant to be installed at Station 3+35 ( lot 49) shall comply with North Andover specifica- tions , consisting of a 6" Mueller or equal gate valves (open left ) , according to AWWA L500-52T regulations . The sliding type cast iron gate box shall be marked "WATER" . b) An 8" ductile iron cement lined water pipe (seal coated inside/tar coated outside ) shall be installed 15 feet from the property line with 5 feet of cover below the finish grade of the street for 335 feet. c) An 8" Mueller or equal gate valves (open left ) is re- quired at the intersection of Fuller and Fuller Meadow Road, d) All work must pass the leakage and pressure tests per- formed in accordance with AWWA Standard 600 and be chlorinated subject to AWWA C-601 Standard Procedure for Disinfecting Water lines . 5 . The entrance to Fuller Meadow Road at Lot No . 51 shall be re- vised to widen the radius and eliminate the reverse curve . 6 . A cross section denoting the elevation at the right-of-way 8" above t{,e gutter line shall be required . 7. The road profile sheet shall be revised to indicate the 28" binder and 26" finish application. 8. Driveway opening permits shall be obtained from the Highway Department. The elimination of driveway berms shall be so depicted on plans per letter dated August 29 , 1983 from the Highway Surveyor . 9 . All easements fully dimensioned on plans shall be submitted in recordable form deeded to the Town of North Andover . 10 . No lot in said subdivision shall be released until Fuller Road is completed and prepared by the developer for acceptance by the Town of North Andover . 11 . A Certificate of Compliance shall be obtained from the Con- servation Commission prior to release of bond or covenant for the completed portion of Fuller Meadow Road . All improvements in said subdivision shall be completed no later than two (2 ) years from the commencement date of con- struction and approval of security .
