The North Andover Planning Board has voted to APPROVE said plan, subject to the

a "covenant running with the land", or otherwise provide security for the con-
struction of wags and the installation of municipal services within said sub-
division, all as provided by G.L. c. ll, S. 81-J.
2. That all such construction and installations shall in all respects
conform to the governing rules and regulations of this Board.
3. That, as required by the North Andover Board of Health in its report to
this Board, no building or other structure shall be built or placed upon Lots
No. 1 through 12 as shown on said Plan without the prior

GREAT POND ESTATES - DEFINITIVE PLAFWeJ1.21LM 1984 and

1. All such construction and installations shall in all respects
conform to the Governing Rules and Regulations of this Board.
Any changes to the approved plan by the developer, town board
or agency shall be submitted to the Planning Board for
approval by the applicant prior to implementatin in the field.
2. A Notice of Intent shall be filed with the North Andover
Conservation Commission in accordance with the Wetlands
Protection Act, G. L. Chapter 131, Section 40, prior to
construction. Any changes necessitated by an Order of
Conditions may be cause for a modification of submission under
Chapter 41 .
3. Prior to endorsement of plans, the applicant shall provide
appropriate street names and draft them on the plan, subject
to approval by the Planning Board.
4. Said dubdivision is subject to Section 4.133, Watershed
District of the North Andover Zoning By Law. The approval of
the definitive plan shall in no way imply blanket approval of
any construction or alteration with the 250 feet buffer zone
of Lake Cochichewick or its tributaries .
5. The construction of the subdivision (including but not limited
to road, utility or dwelling construction) shall be

detailing the construction timetable to be reviewed and
approved by the Planning board prior to signing the definitive

commencement of the second phase of construction .
6. No binder shall be placed without forwarding to the Planning
Board a certification from the applicant ' s engineer listing
and certifying that all utilities have been installed in
accordance with the Rules and Regulations .
7. Lots 5 and 6 shall not be released from the covenant until
Special Permits and/or Variances have been obtained from the
respective boards, in accordance with Section 4. 133 of the
Zoning By Law .

9. All proposed stump pit areas shall be clearly marked on the
definitive plan and approved by the Planning Board and Highway
'Surveyor .
10. The applicant shall submit in writing a request to waive
Section IV,A, j . of the Rules and Regulations concerning the
intersection radius . Appropriate measures shall be taken to
improve the existing site distance. The placement of a Stop
Sign at the proposed road as it approaches Great Pond Road may
be required.
11. No lots or roadway construction shall commence until all
sedimentation basins, compensating storage area(s ) , erosion
controls and barriers have been completed and stabilized. All
erosion measures within the Watershed District shall be bonded
as a separate item to ensure continued maintenance for proper
effectiveness .
12. The applicant shall delineate proposed dwellings , driveways ,
septic system with any proposed lot grading and/or alteration.
Since this development is in close proximity to the Town' s
water supply-Lake Cochickewick , the Planning Board feels that
all lot alteration shall be shown to indicate the impact upon
wetlands immediately adjacent to the Lake. Any lot alteration
between the roadways and wetlands will have a direct effect on
the ability of the wetlands to properly filter out pollutants .
Additional controls shall be considered during construction if
the Board deems necessary.
Erosion controls shall be required across the wetland area on
Lot 10 immediately downstream of any proposed work .
Compensating storage area shall be required to mitigate the
runoff for all storms including the 100 year storm. In larger
storms, surface runoff from Lots 1 - 6 will not be collected
in the drainage system and will flow down the roadway into the
Lake, compensating storage on Lots 8 - 12 shall be of
sufficient capacity to alleviate the increased runoff from the
East side of the subdivision.
13. The edge of the Bordering Vegetated Wetlands shall be
delineated on the plan across Lots 5, 6, and 7 as they abut
the Lake .
14. The sedimentation pond on Lot 6 shall remain intact as a
permanent mitigation device for pollution control. The
drainage easement on Lots 5 and 6 shall be delineated on Sheet
5 of the subdivision plan and shall accomodate all of the side
slopes of said pond.

(4) foot sump will be required in all drainage basins .
17. Eight (8) inch precast concrete units shall be required on all
drainage structures .
18. Slope easements shall be required where necessary to meet
slope and shoulder requirements to be prepared and shown
on the definitive plan by the applicant . All easements
are to be recorded by the applicant and deeded to the
Town of North Andover .

19. Any drainage from cellars or lot development shall not
be emptied into the street. It shall be tied into the
planned drainage system with plans submitted to the
Highway Department for approval .
20. Cape Cod berm shall be required where and when necessary,
to be determined in the field. Road pavement and gravel
width shall be increased by three ( 3) feet in all areas where
berm is necessary.
21 . Subdrainage and any other unforseen drainage problems
picked up during construction shall be incorporated
into the drainage plans , to be determined in the field and
submitted in writing to the Planning Board.
22. Any brick used shall be clay brick. All masonry shall
have solid bed and end joints of good quality cement
mortar .
23. An eight (8) inch ductile iron cement lined water pipe# seal
coated inside, tar coated outside shall be installed for
1, 032 feet and be fifteen ( 15) feet from the property line
with five ( 5) feet of cover below the finish grade of the
street .
24. Eddy breakflange fire hydrants to North Andover speci-
fications are to be installed at Stations 5+40 and 10+32.
-These hydrants shall be installed with a 6 inch Mueller
or equal gate valve (open left) complying with AWWA
specification L500 52T with 100 lb. sliding type cast
iron gate boxes marked "WATER" . An 8 inch gate valve
shall be required at the intersection of Great Pond Road.
-All of the above work must pass the leakage and pressure
tests performed in accordance with AWWA Standard C600
and shall be chlorinated in accordance with AWWA C601
"Standard Procedure for Disinfecting Water Mains" .

25. All fire hydrants shall be .installed at the required 500 foot
