Old Center -- Preliminary Plan
September 9, 1985
Plan dated: June, 1985 Merrimack Engineering revised , September 1985
1 . A Notice of Intent shall be filed with the Conservation
Commission as required under Chapter 131 - Wetland Protection
Act and the local wetlands regulations in accordance with
the determination made on August 30, 1985 .
2. The distance between fire hydrants at Lot #6 and the cul-de-sac
exceeds the required 500' interval.. The definitive plan shall
bring this regulation into compliance.
3. The proposed street name, 0ld Center Street, is not acceptable
by the Fire Department for safety reasons. Two (2) alternative
names shall. be submitted for review and approval prior to or
at the filing of the definitive.
4. A permit is required by the Highway Department prior to any
tie-ins to existing street drainage. Any drainage added to
the Massachusetts Avenue drainage will require an upgrading of
that system.
5. Any drainage from cel.lars ' or .lot development shall be tied
into designed drainage systems and mentioned on the definitive
plan, and approved by the Planning Board and Highway Surveyor.
Drainage not shown on the definitive plans will not be added
during construction unless required by the Highway Surveyor.
6. Any brick used shall be clay brick. All masonry is to have
solid bed and end joints of quality cement mortar. Basin
units to be eight inches precast with cone tops.
7. Roadway cross-sections will be clearly illustrated on plan
for definitive review.
8. Areas to be stumped shall be clearly delineated on the final
definitive plans to be reviewed and approved by the Planning
Board.
9. . The applicant shall submit a Statement of Environmental
Impact as required in .Section , Ill B5 to the Planning Board.
Information describing- in detail the impact of Town services
Subsection E shall be included.
10. Prior to seeking lot releases, the applicant shall have
installed utilities and applied the proper binder coat from
Osgood Street to the proposed ,road, This will ensure safe
access for vehicular travel by residents and emergency
vehicles.
11 . The owner shall retain title to the fee of each street, path
or easement in or appurtenant to the subdivision until conveyed
to the town. Applicant shall maintain and repair the roads
and drainage structures in a manner satisfactory to the
Planning Board.

12. The definitive plan shall clearly delineate the edge of the
bordering Vegetated Wetlands and be flagged on the site.
13. Drainage calculations shall; be submitted to reflect surface
runoff to zero in both the 10 year and 100 year storms.
14. Copies of deed and any mortgage on property showing ownership
to Old Center Street will be submitted with definitive plans.
15. The 20 foot buffer strip between the .proposed road and
abutters of Old Village Lane shall be described on the deed
of Lot 1 as a buffer strip to remain green and unbuilt upon,
to be maintained by said owner.
16. Permission to tie into the drainage system to Massachusetts
Avenue will require permission, from the Town of North Andover
otherwise the tie-in shall connect at the intersection of
Osgood St. and Massachusetts. (no drainage tie-in without
permission of Selectmen and approval form Highway and Planning) .
17. No soil , including gravel will be removed from the site.
18. The grater main on Old Center St. shall be installed 10 feet
right of the centerline of the road. An eight inch Mueller
or equal gate valve shall be installed at the existing 12"
main or. Osgood Street.
Eddy breakflange hydrants to North Andover Specifications
are to be installed at station 100 + 50 and 104 + 60.
19. All water mains shall be eight inch class 52 ductile iron
cement lined water pipe installed with 5 feet of cover
below the finish grade,
20. All hydrant branches shall be provided with six inch Mueller
or equal gate valves (open left) .
21 . . All gate valves shall comply with AWWA specifications L 500
52T and will be supplied with a 100 lb. sliding type gate
box marked "WATER°.,:
22. All water mains shall pass leakage and pressure tests per-
formed in accordance with section 13 of the AWWA Standard
C600 and shall be chlorinated in accordance with AWWA C601
"Standard Procedures for Disinfection of Water Mains. "
23. The sewer main on Old Center St. shall be 8" vitrified clay
pipe installed in the centerline of the road.
24. Precast manholes shall conform to ASTM designation C478
and shall meet the following additional requirements, the
wall thickness shall be not less than 5 inches, sections
shall have tongue and groove joints with an approved bitumastic
gasket. Pipe connections shall be of the rubber boot with
stainless steel band or approved equal. Manhole frames and
covers shall be cast iron lettered "SEWER" and shall be eight
inches high.

25. All sewers must pass low pressure air tests with a maximum
allowable leakage rate of 0.0015 cubic feet per square foot
of internal pipe surface.
26. Adequate public liability and property damage insurance as
well as a street opening bond in an amount satisfactory
to the Highway Surveyor must be provided for the work on
Osgood Street.

Dear Mr . Long :
The Board of Appeals held a public hearing on August 12 , 1986 upon the
application of George B . Stern & Wendy Stern requesting a variance of
Section 7 , Paragraph 7 . 3 and Table 2 of the Zoning ByLaws so as to
permit a variation of sideline requirements from thirty(30) to eighteen
(18) feet . The following members were present and voting : Frank
Serio; Jr . , Chairman , Alfred Frizelle , Vice-chairman , William Sullivan ,
Raymond Vivenzio and Anna O ' Connor .
The hearing was advertised in th.e North. Andgiver Citizen on June 19 and
June 26 , 1986 and all abutters were notified by regular mail. Motion
made by Raymond Vivenzio and seconded by Alfred Frizelle to GRANT this
petition as requested . The vote was unanimous . The Board feels that
the granting of this variance will in no way be a detriment to the
neighborhood.
