The principal points upon which I base my application are as follows: (roust be stated in detail) The Stevens Memorial Library petitioned the voters at the 1986 Town Meeting to approve the construction of a library addition, which they unanimously approved. The legal notice was duly advertised in the North Andover Citizen on March 31, and April 7, 1993 and all parties of interest were properly notified. The following members were present: Joseph Mahoney, Richard Rowen, John Draper, John Simons and George Perna. The petitioner was requesting site plan approval for the construction of a 10,200 sq, ft, addition to the existing library. Mr. Mahoney read the legal notice to open the public hearing. Ms. Bradley discussed the Technical Review Committee comments and the parking issues. Mr. Mahoney read Mr. Richard Doucette's comments. Arthur Larson, Chairman of the Building Committee was present. Others members of the Building Committee present were Joanne Stevens, Mary Ellen Osgood, and Sue Ellen Holmes. The following architects and engineers were present, Dick Thuma, Tad Stahl, and Frank Adams. Tad Stahl gave a presentation of the model. 1. Larches on site will be removed under current plan. 2. Want building to look out onto park, not be blocked. 3. Want to keep plantings minimal so that it looks out onto the park. 4. Eliminate former front entry way, brick path. Page 2: Stevens Memorial Library 5. Widen road 10 feet into site to make room for angle parking. 6. Emergency vehicle access is provided. 7. Two handicap spaces are provided. Mr. Simons questioned the building material. Will it match current building materials? The answer was yes and that the air conditioning system will be hidden by roof structures. Mr. Draper questioned if there was any other outside lighting than shown on the model. The answer was "no" but some could be added. Mr. Simons questioned the landscape plan. Landscaping was as shown on plans. Mr. Draper questioned the safety of kids who may wander away from the outside area even under supervised activities. Mr. Simons asked for input from abutters. There were none present. Mr. Draper stated that he liked the sky light adaption, but had concerns about longevity. Mr. Rowen commended the design. Mr. Rowen had the following items: 1. Plan for construction - schedule is needed. 2. Plan to keep library open during construction? 3. Any roads shut down during construction? No plans to shut any streets down. 4 . Construction material storage plan is needed. On a motion by Mr. Mahoney, seconded by Mr. Draper, the Board voted to close the public hearing. The decision will be discussed at the May 18, 1993 meeting. On May 18, 1993 the Planning Board held a regular meeting in the Senior Center behind the Town Building. The following members were present: Richard Nardella, Joseph Mahoney, Richard Rowen, George Perna, and John Simons. Ms. Bradley read a letter from Mr. Doucette, Conservation Administrator, requesting that a condition be placed in the decision regarding the protection of larches on the site. The Board decided to include Mr. Doucette condition into the decision. The Board also decided to keep the brick walkway at 8 ft. and not expand it to 9 ft. as suggested by the Fire Department. Page 3: Stevens Memorial Library On a motion by Mr. Mahoney, seconded by Mr. Rowen the Board voted to approve the draft with the amendments. Nardella, Chairman cc: Director of Public Works Building Inspector Conservation Administrator Health Agent Assessors Police Chief Fire Chief Applicant Engineer File STEVENS MEMORIAL LIBRARY 345 MAIN STREET SITE PLAN REVIEW CONDITIONAL APPROVAL The Planning Board herein conditionally approves the Special Permit requested by the Town of North Andover, Stevens Memorial Library, dated March 26, 1993 for the construction of an approximately 10, 000 square foot addition to the south side of the existing library. The Planning Board makes the following findings as required by the Town of North Andover Zoning Bylaw Section 10. 3 Special Permit: A. The specific site is an appropriate location for the addition to the library as the library already exists on the site; B. The use as developed will not adversely affect the neighborhood as an addition to the current library will not have any greater impact than its current use; C. There will be no nuisance or serious hazard to vehicles or pedestrians as the parking layout has been approved by the Police Station and the Department of Public Works; D. Adequate and appropriate facilities will be provided for the proper operation of the proposed use; E. The Planning Board finds that the use is in harmony with the general purpose and intent of this bylaw. The Planning Board finds that this proposal complies with the Town of North Andover Zoning Bylaw Section 8.3 Site Plan Review under the conditions set forth below. The Planning Board hereby GRANTS site plan review approval to the applicant provided the following conditions are met: 1. Prior to endorsement of the plans by the Planning Board and recording with the Registry of Deeds, the applicant shall adhere to the following: a. All Conditions listed herein shall be placed on the final sheet of the recordp lans. b. The space marked "Emergency Only" on the plans shall be designated and marked as a "Fire Lane" in accordance with the North Andover Fire Department. C. The applicant must obtain all required variances from 1 the Zoning Board of Appeals. 2 . Prior to FORM U ve rificationbuildin permit issuance verification (building P ) a. This decision shall be recorded at the Essex North Registry of Deeds and a recorded copy delivered to the Planning Staff. 3 . Prior to verification of the Certificate of Occupancy: a. the building shall have fire sprinklers installed per order of the North Andover Fire Department. b. the applicant must install a fire alarm system with a new control panel, annunciator and all appurtenances as required by the North Andover Fire Department. C. Existing underground fuel oil tanks must be removed. The applicant will work with the Fire Department as to the timing of the removal of the tanks. d. a supra box must be installed at a location to be designated by the Fire Department e. the site shall have received all necessary permits and approvals from the North Andover Board of Health. f. the site shall be reviewed by the Planning Board. Any screening as may be reasonably required by the Planning Board shall be added at the applicant' s expense. g. A final as-built plan showing the location of all on site utilities, structures, curb cuts, parking spaces and drainage facilities be submitted to and approved by the Division of Public Works. h. All artificial lighting used to illuminate the site shall have underground wiring and shall be so arranged that all direct rays from such lighting falls entirely within the site and shall be shielded or recessed so as not to shine upon abutting properties or streets. 4 . The contractor shall contact Dig Safe at least 72 hours prior to commencing any excavation. 5. Gas, telephone, cable and electric utilities shall be installed as specified by the respective utility companies. 6. All catch basins shall be protected and maintained with hay bales to prevent siltation into the drain lines during construction. 2 7. No open burning shall be done except as is permitted during burning season under the Fire Department Regulations. 8. All building material used in construction of the addition shall be consistent with the material used for the existing library. 9. The applicant, in conjunction with the Town Planner and/or Tree Warden, shall mark all trees to be preserved prior to any construction on site. 10. The applicant shall meet with the Conservation Administrator/Deputy Tree Warden within 30 days of the filing of this decision with the town clerk to determine the feasibility of preserving some of the mature larch trees on site. 11. Any plants, trees or shrubs that have been incorporated into the Site Plan/Landscape Plan approved as a part of this decision the die within one year from the date of planting must be replaced by the applicant. 12 . The provisions of this conditional approval shall apply to and be binding upon the applicant, its employees, and all successors and assigns in interest and control. ACHUS TOWN OF NORTH ANDOVER MASSACHUSETTS BOARD OF APPEALS Town of North Andover/ DECISION Stevens Library Trustees 345 Main Street Petition #022-93 North Andover, MA 01845 The Board of Appeals held a regular meeting on Tuesday, May 11, 1993 upon the application of the Town of North Andover/Stevens Library Trustees requesting a variation of Section 8, Paragraph 8. 12 of the Zoning Bylaws so as to permit relief of parking, 29 parking spaces in lieu of 44 spaces. The following members were present and voting: Frank Serio, Jr. , Chairman, William Sullivan, Vice-chairman, Walter Soule, Clerk and John Pallone and Robert Ford. The hearing was advertised in the North Andover Citizen on April 21 and 28, 1993 and all abutters were notified by regular mail. Upon a motion by Mr. Sullivan and seconded by Mr. Ford the Board voted unanimously to GRANT the variance as requested.
