Dear Michelle,
We have completed a septic system upgrade design for the above referenced site.
Enclosed herewith is a copy of the Septic Plan Submittal Form and fee,the Soil
Evaluation Forms,the Particle Size Analysis Report by Terra Filter, a Local Upgrade
Approval Form, and the Infiltrator Chamber Technology Certification along with 3
copies of the plans.
Due to the overwhelming predominance of wetlands on site,the system as designed,
requires a waiver from the NA Board of Health Regulations to allow the septic tank to be
50 feet from a wetland where 75 feet is required.
On behalf of the owner, Doug Schmidt,we respectfully request this matter be placed on
the earliest available BOH meeting agenda for consideration of the waiver request.

Re: Subsurface Sewage Disposal System Plan for 526 Winter Street(Map 104A,Lot 80)
Dear Mr.Nemchenok:
The proposed wastewater system design plan for the above site dated May 18, 2015 and received
on May 21, 2015 has been reviewed. Unfortunately,the plan cannot be approved until the
following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North
Andover regulation that is not met by this design follows each item.
1. Cleanouts are required at all bends in the building sewer line(3 10 CMR 15.222(8)).
2. Since the Infiltrator Chamber system is proposed as an alternative soil absorption system
the"Standard Conditions for Alternative Soil Absorption Systems with General Use
Certification and/or Approved for Remedial Use" will apply. Please provide the
following as required by the approval conditions
Section II(7):
e) The record draivings, approved by the LAA, must clearly indicate an area for the best
feasible replacement system that could be installed in the event that the proposed
Alternative Soil Absorption System fails or it is determined that it is not capable of
providing equivalent environmental protection;

Dear Mr Chairman and Members:
We have completed an upgrade septic system design for the above referenced site. The
site poses many challenges and has many pre existing conditions in the rear yard that are
not necessarily shown on the drawing such as in ground swimming pool, walkways,
landscape features, etc., and resulted in a replacement system in the front of the house.
Soil conditions in front are favorable but with a high water table resulting in a raised
system.. In order to maintain adequate flow of storm water runoff and not cause ponding,
damming or trapping of surface water, it was necessary to design the system with a
request for LUA to allow the system to be 4 feet above ground water where 5 feet is
required for full compliance.
On behalf of the owner, Jack Burns, I respectfully request this matter be placed on your
June 25th meeting agenda for consideration of this Local.Upgrade Approval request.

Dear Mr.Nemchenok:
The proposed wastewater system design plan for the above site dated June 1,2015 and received
on June 2, 2015 has been reviewed. Unfortunately, the plan cannot be approved until the
following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North
Andover regulation that is not met by this design follows each item where applicable.
1. Please confirm with Conservation that there are no wetland resource areas within 150'
from the proposed tanks and leach field (NA 3.2).
2. Indicate the location of the existing leaching facility (NA 3.2).
3. Since the Infiltrator Chamber system is proposed as an alternative soil absorption system
the"Standard Conditions for Alternative Soil Absorption Systems with General Use
Certification and/or Approved for Remedial Use"will apply. Please provide the
following as required by the approval conditions

the Owner's Manual, and the Operation and Maintenance Manual,

2. for Systems installed under a Remedial Use Approval, the owner
agrees to fulfill his responsibilities to provide written notification
of the Approval to any new Owner, as required by 310 CMR
15.287(5);
3. if the design does not provide for the use of garbage grinders, the
restriction is understood and accepted; and
4. whether or not covered by a warranty, the System Owner
understands the requirement to repair, replace, modify or take any
other action as required by the Department or the LAA, if the
Department or the LAA determines the System to be failing to
protectpublic health and safety and the environment, as defined in
310 CMR 15.303.
