Provide easements for the common driveways,water and sewer utilities,drainage,retaining walls and other common or shared features.These easements need to provide for maintenance,repair,and inspection. Given that these items are in the Watershed Protection District,these easements must also provide access for inspection and repair of these systems by the Town should the owners fail to provide repairs deemed necessary by the Division of Public Works in support of health or safety issues. Provide driveway entrance details,grading plan and proposed radii for driveway curves at the intersection with Bradford Street. Drainage: Drainage calculations and pipe sizing are required for the 12"site drainage piping. Drainage calculations and basin sizing is required for sizing of leaching basin. Drainage calculations and pipe sizing are required for the 12"driveway culvert pipes at Bradford Street. Revise driveway profiles to insure that no driveway runoff directly enters onto Bradford Street. Provide drainage calculations,construction details and outlet destination for the runoff to be discharged into the ditch/swale adjacent to Bradford Street. The special permit submission was reviewed for conformance to the appropriate sections of the 1972 Town of North Andover Zoning Bylaw as amended May 2001 and standard engineering practice. The following comments note non-conformance with specific sections and questionstcomments on the proposed design. VHB offers the following comments: Section 2,30.1 Common Driveway—Lot 3&Lot 4 The Applicant should verify that a maintenance agreement between Lot 3 and 4 for the common driveway has been submitted to Planning Broad for review.Also,will a common driveway be required for Lot 1 and 2? Sec"4.136.4 Watershed Protection District 1. (4.c.iii)This section requires that a written certification regarding the quality and quantity of water entering Lake Cochichewiek should be made by a registered Professional Engineer or a scientist educated in and possessing extensive experience in the science of hydrology and hydrogeology.It appears that a written certification in the special permit application was made by a registered Landscape Architect,not by a registered Professional Engineer.VHB recommends that a written certification be wade by a registered Professional Engineer. 2. (4.c.iv) This section requires that a reasonable alternative location outside of the Non-Disturbance or Non-Discharge zone be investigated. It appears that the Applicant could locate the proposed houses on Lot 3 and 4 in the Town of Boxford outside the Non-Discharge zone.The Applicant should justify why there is no reasonable alternative location outside the Non-Discharge zone for the proposed houses. 3. (4.c.vi)It is not clear how the Applicant proposes to limit the application of fertilizer by private homeowners through the conditions of Special Permit Approval.Will the conditions of Special Permit Approval be included in the property deed?VHB requests that additional explanation be provided. Section?Dimensional.Uguwements It appears that Lot 1 and 2 do not meet the minim=frontage dimension of 175 feet.Has a frontage exception been submitted to Planning Board for approval?For clarity,VHB recommends that the entire parcels of land be shown on the larger scale of plans. Andover, MassachuseLls 01845 Telephone(978)688-9535 Fax(978)688-9542 MEMORANDUM TO: North Andover Planning Board FROM:Jacki Byerley,Interim Assistant Town Plannell RE: Lot 3 Bradford Street-Watershed&Common Driveway Special Permit DATE: May 14,2002 This proposal is for a single family home and common-driveway to be shared with the adjacent Lot 4 located within the Non-Discharge Zone. An ANR plan was submitted on May 22,2001 creating the lot. My review is as follows: 1. The driveway entrance and dwelling will be located in North Andover. The parking portion of the driveway is located in Boxford, the developer should state whether a permit will be required from Boxford for this. 2. The applicant proposes to have water and forced main line from Bradford Street to service the home. 3. The lot has a total square footage of 756,549 within the R-1 Zoning District. The requirement for that district is 87,120 SF. 4. A single-family dwelling and shared driveway are allowed uses with the R-1 Zone. 5. A roof infiltration system is proposed at the front of the home. 6. The applicant has demonstrated that clearing of the lot is kept to a minimum to cause less alteration to the site as a shared driveway is proposed. 7. Wetlands have been Ragged and haybales are proposed along the limit of clearing and outside of the Non-Disturbance Zone. 8. Catch basins and an infiltration trench are proposed. 9. An easement and maintenance agreement needs to be provided for the common driveway. 10. The applicant needs to demonstrate how far from the annual mean high water mark of Lake Cochichewick the lot is. The applicant has stated on the plans that the lots are located in the Flood Plain District Zone A in accordance with Section 4.137(5)a. the applicant must provide certification by a registered professional engineer that the construction will not result in any increase in flood levels during the 100 year flood with the Building Commissioner and Director of DPW in agreement. 130AR 1)OF APP FALS x"88-9541.
