In accordance with the provisions of M.G.L. Chapter 40-A, Section 11, the North
Andover Planning Board will hold a public hearing as follows:
Purpose of Public Hearing: Application for a Watershed Special Permit under
Section 4.136, a Common Driveway Special Permit
under Section 2.30.1 and an Access Across Street
Frontage Special Permit under Section 7.2.1 of the
North Andover Zoning Bylaw to permit the construction
of a single family home, access to shared driveway,
utilities, fieldstone walls and site grading.

The Applicant submitted a plan,special permit application and supporting documents to VHB for review.
The special permit submission was reviewed for conformance to the appropriate sections of the 1972 Town
of North Andover Zoning Bylaw as amended May 2001 and standard engineering practice. The following
comments note non-conformance with specific sections and questions/comments on the proposed design.
VHB offers the following comments:
Section 2,30.7 Common Driveway—Lot 3&Lot 4
The Applicant should verify that a maintenance agreement between Lot 3 and 4 for the common driveway
has been submitted to Planning Broad for review.Also,will a common driveway be required for Lot 1 and
2?
Section 4.136.4 Watershed Protection District
1. (4.e.iii)This section requires that a written certification regarding the quality and quantity of water
entering Lake Cochichewick should be made by a registered professional Engineer or a scientist
educated in and possessing extensive experience in the science of hydrology and hydrogeology.It
appears that a written certification in the special permit application was made by a registered
Landscape Architect,not by a registered Professional Engineer.VHB recommends that a written
certification be made by a registered Professional Engineer.
2. (4.c.iv) This section requires that a reasonable alternative location outside of the Non-Disturbance or
Non-Discharge zone be investigated. It appears that the Applicant could locate the proposed houses on
Lot 3 and 4 in the Town of Boxford outside the Non-Discharge zone.The Applicant should justify why
there is no reasonable alternative location outside the Non-Discharge zone for the proposed houses.
3. (4.c.vi)It is not clear how the Applicant proposes to limit the application of fertilizer by private
homeowners through the conditions of Special Permit Approval.Will the conditions of Special Permit
Approval be included in the property deed?'VHB requests that additional explanation be provided.
Section 7 Dimensional_Rearrirements
It appears that Lot 1 and 2 do not meet the minimum frontage dimension of 175 feet.Has a frontage
exception been submitted to planning Board for approval?For clarity,VHB recommends that the entire
parcels of land be shown on the larger scale of plans.
Drainar,e Comments
It is not unclear whether test pits or percolation tests have been performed to verify existing soil infiltration
rates and existing groundwater elevation.Since the majority of the driveway is in a"cut section',VM
recommends that test pits and percolation tests be performed to verify that the proposed driveway
infiltration trench system will function as designed.

The Division of Public Works has reviewed the subject plans dated April 10, 2002 for Lot 2: Watershed Special
Permit, Lot 3: Watershed Special Permit and Common Driveway Special Permit, and Lot 4: Watershed Special
Permit, Common Driveway Special Permit, and Access Across Street Frontage Special Permit, and offer the
following comments:
General:
Provide easements for the common driveways,water and sewer utilities,drainage,retaining walls and other common or
shared features.These easements need to provide for maintenance,repair,and inspection. Given that these items are in the
Watershed Protection District, these easements must also provide access for inspection and repair of these systems by the
Town should the owners fail to provide repairs deemed necessary by the Division of Public Works in support of health or
safety issues.
Provide driveway entrance details, grading plan and proposed radii for driveway curves at the intersection with Bradford
Street.
Drainage:
Drainage calculations and pipe sizing are required for the 12"site drainage piping.
Drainage calculations and basin sizing is required for sizing of leaching basin.
Drainage calculations and pipe sizing are required for the 12"driveway culvert pipes at Bradford Street.
Revise driveway profiles to insure that no driveway runoff directly enters onto Bradford Street.
Provide drainage calculations,construction details and outlet destination for the runoff to be discharged into the ditch/swale
adjacent to Bradford Street.
Provide construction details for the proposed foundation drainage.
