Dear Mr. McIntosh:
Per our telephone conversation on Wednesday, September 27, 2000, this
correspondence addresses your seven (7) comments regarding the above
referenced site, as follows:
1. Enclosed is a copy of the deed siting the prior owner's
deed, dated January 25, 1994, prodating October 24, 1994.
2. We agree.
3. Erosion control measures are shown on the enclosed revised
plan.
4. Engineer's stamp is on plan.
5. Enclosed find copies of the assessor's and flood plain maps,
showing the subject premises being 900 feet and 800 feet,
respectively. Also, the lake elevation is at elevation
116+ feet, and the subject addition is at elevation 175+ feet,
which is 59+ feet above the lake.
6. All zones are now shown on the plan.
7. Per subsection 4g, a roof drain is designed to recharge the
300 square feet roof surface runoff into a leaching pit, as
recommended by DEP Storm Water Management 'Progra,m.
Hopefully, all comments have been addressed to your satisfaction.

REVIEW OF SPECIAL PERMIT APPLICATION
FOR CONFORMANCE WITH THE TOWN OF NORTH ANDOVER
ZONING BYLAW&STANDARD ENGINEERING PRACTICE

The Applicant submitted a site plan,special permit application and cover letter to VHB for review.The
special permit submission was reviewed for conformance to the appropriate sections of the 1972 Town of
North Andover Zoning Bylaw reprinted in 1998 and standard engineering practice. The following
comments note non-conformance with specific sections and questions/comments on the proposed design.
VHB offers the following comments:
Section 4.136 Watershed Protection District
1. VHB assumes that this lot was created before October 24, 1994 and therefore the criteria in Table 2 of
subsection 21 will apply. The Applicant's engineer should confirm when the lot was created.
2. Based on Table 2,the proposed addition falls within the Non-Discharge Buffer Zone.
3. Subsection 3.b.iv.requires that construction within the Non-Discharge Buffer Zone comply with best
management practices for erosion,siltation and stormwater control in order to preserve the ground
water and the lake. No best management measures are apparent on the plan. VHB recommends that
erosion control measures like hay bales and silt fences be provided on the site plan.
4. The site plan was not stamped with a registered professional engineering stamp. The Applicant's
engineer should stamp the plan.
5. Subsection 4.c.ii.1.requires that the annual high water mark of Lake Cochichewick be shown on the
plans if the proposed activity is within 400 feet of the annual high water mark. It appears that the
proposed addition is within 400 feet of Lake Cochichewick. The Applicant's engineer should either
show the annual high water mark on the plan or confirm that the proposed activity is not within 400 feet
of the annual high water mark.
6. Subsections 4.c.ii.3.,4.and 5.require that the Conservation Zone,Non-Disturbance Zone and Non-
Discharge Zone be shown on the plan. VHB recommends that the Applicant' engineer show these lines
on the plan.
7. Subsection 4.g.requires that runoff from impervious surfaces,to the extent possible,be recharged on
site. VHB recommends that runoff from the roof of the proposed addition be directed to an
underground leaching pit or chamber so that the runoff can infiltrate to the ground water.

plan(dated 9-28-00)which responds to our Engineering Review for the above referenced
project. It appears that all of VHB's comments have been adequately addressed and VHB's
concerns in this matter have been satisfied.
