Dear Commission Members,
We are hereby requesting a.Certificate of Compliance in accordance with Condition 471 of an
Order of Conditions issued on November 6, 2014 for the above project. The work has been
satisfactorily completed in compliance with the approved plan and the Order of Conditions, with
the following notable discrepancies/observations.
- The as-built deck is smaller than the deck shown on the approved plan at the rear of the
house.A pervious paver patio was added beneath the deck,instead of crushed stone
surface indicated on the approved plan.
- The paved driveway was constructed somewhat differently than as shown on the
approved plan. A narrow turn-around area was added to the end of the driveway however
the area by the rear garage corner was reduced. Also a paved walkway was added
connecting the driveway to the deck stairs. Overall the as-built impervious paved surface
within the 100ft. buffer zone is equivalent to the area on the approved plan.
Please find enclosed a completed Form 8A and an As-Built Plan. The site has been graded in
compliance with the approved plan.Disturbed areas have been stabilized with grass lawn and
landscaping.
We ask that this request be placed on the agenda of the earliest available Conservation
