The Order of Conditions was issued to the Kutz's for this project and they are still the current landowners. They have read and understand the recorded Order of Conditions. The Certificate of Compliance is to be granted to Jeff&Alyson Kutz. As part of this request,I am attaching a completed WPA Form 8A application,a check for$200.00, and 2 full size As-Built plans&7 reduced sized plans (for the Commission). I personally inspected the site on May 1,2015 and found the site to be fully stabilized by means of grass and other landscaped materials. Wetland emblems were in place along the 25 foot No Disturbance buffer zone. The remains of the haybales and silt fencing are still in place and should be removed upon approval of the Conservation Agent to do so. The site was constructed in substantial compliance with the Order of Conditions and the approved plans with the following minor changes: 1) A proposed patio was approved to built to the rear of the proposed addition. A smaller deck was constructed in this location in lieu of the paver patio. 2) A propane tank and AC unit were installed on the westerly side of the addition that were not shown on the approved plans. These items are approximately 65 feet away from the wetland resource area. 3) A landscaped boulder retaining wall(<2 feet high+20 feet long)was constructed on the northwesterly side of the recent addition that was not shown on the approved plan. The wall is approximately 58 feet from the wetland.resource area at the closest point.. 4) The front walkway on the approved plan was replaced with a paver walkway in a different configuration than the original walkway.
