RE: Title V second review for 39 Hawkins Lane
Dear Sandra,
('This review is of design plans with revision date of 5/16/2000 in which the leaching
bed has been changed from two trenches to three trenches.)
Enclosed find the"Checklist for North Andover Septic System Plans" for the above-
mentioned site. The following is a list of all the `Problem' areas and deficiencies fort
Engineering has found.
o The actual observed estimated seasonal high water elevation must be adjusted for
the high point of the leaching bed. The high point of Trench#I is 124,5'. An
ESHW at 104 inches gives an elevation of ESHW of 115.8'. The bottom of system
must be 4 feet above or at elevation 11.9.8'. The design plans show the elevation of
bottom of Trench#I to be 117'. Thus'Trench 41 must be raised 2.8 feet. In like
manner,Trench#2 must be raised by 0.3 feet. Trench 43 is adequately graded_
(Note that as designed, the final grade over"Trench#1 is 120.5', yet the grade of the
proposed parking area only a foot away is shown as 124', or a 3.5 foot rise in only
one foot of run.)

3. Fee of JU-6._0Q per lot for DM construction. This covers the minimum two deep holes
and two percolation tests required for each disposal area. Fee of$Z5.00 per lot for
reDaiirs or uggrades.
GENERAL INFORMATION
i
1. Only Certified Soil Evaluators may perform deep hole inspections.
2. Only Mass. Registered Sanitarians and Professional Engineers can design septic
plans.
3. At least two deep holes and two percolation tests are required for each septic system
disposal area.
4. Repairs require at least two deep holes and at least one percolation test, at the
discretion of the BOH representative.
5. Full payment will be required for all additional tests within two weeks of testing.
6. Within 45 days of testing, a scaled plan (no smaller than 1"-100') shall be submitted to
the Board of Health showing the location of all tests (including aborted tests).
7. Within 60 days of testing soil evaluation forms shall be submitted.
