This Phase I Report describes subsurface investigation activities which have been completed by New England Environmental Technologies Corporation (NEET) on behalf of the property owner, Galaxy LLC, to support the development of a Tier Classification for the site as established at 310 CMR 40.0500 and the completion of a Numerical Ranking System scoresheet as established at 310 CMR 40.1500. NEET was initially retained by the property operator (Central Metal Finishing, Inc.)/owner (Galaxy, LLC)to conduct subsurface investigation activities as part of a real estate assessment of the property located at 80 Flagship Drive in North Andover, Massachusetts (hereinafter referred to as the site). As part of these investigations, three (3) groundwater monitoring wells (MW-1, MW-2 and MW-3) were installed at the site and groundwater samples were analyzed for volatile organic compounds (VOC), in addition to other parameters. A measured concentration of the chlorinated volatile organic compound (CVOC) Trichloroethene (TCE) was detected at 857 micrograms per liter (µg/1) in one (1) of the groundwater monitoring wells (MW-2) above the MCP GW-2 Reportable Concentration (RC) of 300 µg/1 (at that time, prior to the April 3, 2006 revisions to the MCP, the current MCP GW-2 RC is 30 µg/1). The results of these investigation activities were presented to Ms. Carol Shibles, President of Central Metal Finishing, Inc. (CMF) and Managing Member of Galaxy, LLC, in a Level I Environmental Site Assessment (ESA) & Limited Subsurface Investigation (LSI) Report dated June 30, 2005 which was hand delivered on July 19, 2005. The LSI Report also included a municipal file review and a on-line database search of Federal and State records relative to the use, storage, and release of oil and hazardous materials within a one mile radius of the site. 313'fain Street, -ovefar.d, MR 61834 tei:hone (978) 521-111' Facsimile(978)521-1760 D ® October 31,2006 NEET Project #0505502B RTN 3-25399 NEET c T Page 2 of 3 1 GNEET was subsequently engaged by the property owner and Potentially Responsible Parry (PRP), Galaxy, LLC, to further assess the site. NEET coordinated the services of a commercial drilling subcontractor and observed the installation of three (3) additional borings/wells (B4/MW-4, B5/MW-5 and B6/MW-6) at the site property. Boring B-4 was located directly adjacent to well MW-2 and was advanced to a depth of 24 feet below grade where hollow stem auger refusal was encountered. NEET made the assumption that auger refusal was encountered at the surface of bedrock and set the bottom of a 5 foot long well screen at a depth of 24 feet below grade to determine whether or not there was evidence of a dense non-aqueous phase liquid (DNAPL) and/or an increase in the measured concentration of WE contamination to groundwater with increased depth. A measured concentration of TCE was detected at all three (3) newly installed wells but only the measured concentration of TCE reported at well MW-5 at 1,500 µg/1 (located approximately 60 feet downgradient from well MW-2) was above the MCP GW-2 RC of 300 µg/1 and the MCP Method 1 GW-2 Groundwater Standard of 300 µg/1 (prior to the April 3, 2006 revisions to the MCP, the current MCP GW-2 Groundwater Standard is 30 µg/1). The results of these supplemental subsurface investigation activities were presented to Ms. Shibles, in a LSI Report dated September 16, 2005. NEET was subsequently engaged by Galaxy, LLC to provide Massachusetts Licensed Site Professional (LSP) services to report and appropriately assess and manage the apparent release of TCE at the site. On behalf of Galaxy, LLC, NEET prepared a Massachusetts Department of Environmental Protection (DEP), BWSC Release Notification Form (RNF) BWSC-103 and written summary of the observed site conditions associated with the apparent reportable release of hazardous materials observed in groundwater at the above referenced property in accordance with the MCP at 310 CMR 40.0315(2)(3). The BWSC-103 RNF Form and attached cover letter dated November 8, 2005 were mailed to DEP by US Postal Service certified mail signed return receipt requested. DEP subsequently issued a Notice of Responsibility (NOR) letter dated December 6, 2005 and assigned Release Tracking Number(RTN) 3-25399. Additional Phase I subsurface investigation activities conducted by NEET at the site on behalf of Galaxy, LLC included the following: the advancement of three (3) additional test borings (B7/MW-7, B8/MW-8 and B9/1\4W-9) which were completed by the installation of permanent groundwater monitoring wells MW-7, MW-8 and MW-9; collection of soil samples from borings B-7, B-8 and B-9 for VOC headspace screening; collection of soil samples from borings B-7, B-8 and B-9 which were submitted for laboratory analysis of VOCs by EPA Method 8260B; collection of confirmatory groundwater samples from wells MW-1, MW-2, MW-5 and MW-6 which were submitted for laboratory analysis of VOCs by EPA Method 8260B; collection of groundwater samples from wells MW-7, MW-8 and MW-9 which were submitted for laboratory analysis of VOCs by EPA Method 8260B; and the completion of a well head elevation survey and comprehensive depth to groundwater gauging round at all on-site groundwater monitoring wells to determine groundwater flow direction beneath the site. October 31,2006 NEET Project #0505502B RTN 3-25399 NEET Page 3 of 3 At this time, only the CVOC TCE has been reported above an applicable MCP Method GW-2 Groundwater Standard in groundwater collected from four(4) of the nine (9) on-site groundwater monitoring wells (MW-2, MW-5, MW-6 and MW-8). Additionally, the initial measured concentration of TCE reported in groundwater collected from well MW-5 was observed to have decreased to below the detection limit in a subsequent confirmatory groundwater sample. Refer to the attached Figure 2 - Site Sketch for the approximate locations of the on-site groundwater monitoring wells. No other oil and/or hazardous materials have been reported in site media at or above, an applicable MCP S-1 GW-2/GW-3 Soil Standard or MCP Method 1 GW-2/GW-3 Groundwater Standard. At this time the source of the TCE contamination to soil and groundwater is unknown; however, TCE was reportedly used during regular operations at the site property in the past. Additionally, it also appears that TCE was either previously or is currently used at the adjacent upgradient property located at 70 Flagship Drive. Based on the information gathered during this investigation, it is NEET's opinion that evidence of a reportable release of oil or hazardous materials was observed in on-site groundwater. Though the reported measured concentration of TCE in the second groundwater sample collected from well MW-5 was below the detection limit and confirmatory groundwater samples have not yet been collected from wells MW-1, MW-4,MW-7, MW-8 and MW-9, observed site conditions continue to indicate that the site is out of compliance with MGL Ch. 21 E or the MCP 310 CMR 40.0000 relative-to the continued presence of TCE contamination to groundwater. Further MCP response actions are necessary at the site to address the apparent on-site release of TCE which has negatively impacted groundwater. Based on currently available information, Galaxy, LLC anticipates that the release will be managed by conducting additional site assessment activities through the phased MCP process.
