(503) -386-38 2 FIRE CHIEF'S REPORT INCIDENT 80 FLAGSHIP DRIVE - CENTRAL INDUSTRIAL LABS. DATE OF INCIDENT 12/20/92 DATE OF REPORT 12/21/92 The Fire Chief received a page from fire dispatch and upon calling the Chief was informed that Lt. Melnikas requested that he respond to 80 Flagship Drive.- Central Industrial Labs. A fire had occurred in a vat of copper cyanide. Smoke for the deterioration of the plastic vat and vent stack had apparently tripped the smoke detector and transmitted the fire alarm box. While responding the Chief was informed that the fire was out. Upon arrival the Chief met with Lts. Melnikas and Martineau. The fire had been contained to the area of the vat and the adjacent exhaust piping. A quantity of the contents of the vat mixed with the water from the hose line was on the floor in the work area. None of the liquid material had entered the drains and was confined to the dike work area. The liquid was brownish in appearance. Lt. Melnikas informed the Chief that he and firefighter Garvey had entered the room and using, an inch and three quarter hose line had extinguished the fire. The Chief directed firefighter Wentworth to use C-1 to obtain his camera equipment, and had firefighter Garvey conduct the fire investigation. The Chief ordered the inch and three quarter line from the building since it was not required and some of the couplings were leaking in the hallway adjacent to the work area. The Chief then directed that the following agencies be contacte& to come to the scene: the building inspector, the electrical inspector, someone from the Board of Health and DPW. Lt. Melnikas and Firefighter Garvey were utilizing a phone at the facility to contact a response person from the facility. Of the Town agencies contacted only the wiring inspector, John Thompson and Dr. Rizza from the Board of Health arrived at the scene. The DPW director, George Perna, did call back to fire dispatch but at the direction of the Fire Chief was told he need not respond. DPW was called relative to cross connections in the sewer, but the Chief ascertained that the system was a closed loop. The Chief ordered that Lt. Melnikas and FF Garvey remove their protective pants and isolate the length of hose used in the work area. This was to limit skin contact with the then unknown material . These protective pants and hose were sent to National Safety Clean in Pennsylvania for decon. Firefighter Garvey's gloves, nomex hood wool mittens and rut)t,(ir hots were replaced as the result of direct exposure to the chemi(-a_1s. Lt. Melnikas' gloves were replaced. Each man was provided with rer-erne protective pants. Contaminated was plticc-d in plastic bags and transported to the Fire Station. These two personnel were the only two directly exposed to the liquid as they had knelt in the fluid during the extinguishment of the fire. All personnel who entered the building were required to fill out a hazardous material exposure, form. Facility personnel arri'/ed and with the assistance of Mr. Frank Freitis were given the cr,pies of the MSDS sheets for the chemical involved. These were copied at the scene (three copies) one for the Chief, one for fire investigation and one for Dr. Rizza. The Chief had released all companies from the scene with the exception of Engine 2 which remained on scene to conduct the investigation. Lt. Casale on the night shift arrived and informed Mr. Freitis to contact DEP. I.t. Casale is the Right to Know Officer for the department and will k)e filing a separate report, of which this report will be a part. The Chief informed Mr. Freitis that the company would be responsible for cleanup of the material and those personnel were on scene and began the process. He wiir; also informed that the company would be billed for the cleaning and replacement of firefighting equipment cleaning and replacement under 21E. The Chief reset the fire alarm panel. At this point the Chief (_eft the scene with Lt. Melnikas in overall command and FF Garvey in charge of the investigation. END OF REPORT December 21 , 1992 WILLIAM V. DOLAN FIRE CHIEF 1 i I I have called the _Northe,�st regional office of D.E.P. on December 22, 1992 and gave a c®mplete description of the events that took place on December 20, 1992 to Mr. Mike Gorrasi and was told no additional steps had to be taken and that the information was on file. Finally, as a means of corrective action, Central has determined heated tanks will not be left "on" or unattended while the facility is not in a production shift and additional research into liquid level float switches will be forthcoming to automatically control on/off functions of all heaters in process tanks. METAL FINISHING FOR THE AEROSPACE AND RELATED INDUSTRIES December 22, 1992 Mr. Joel Weddig Massachusetts Department of Labor and Industries Division of Occupational Hygiene 1001 Watertown Street West Newton, MA 02165 Dear Joel, Per your request I am sending you this letter to confirm our telecon of December 22, 1992 and review the incident of December 20, 1992 at Central Metal Finishing, Inc. . At approximately 4:00 p.m. December 20, 1992, I was advised by my Emergency Response Coordinator that a fire had occurred at the facility (no personnel were present at the time) and the North Andover Fire Department required our assistance in determining cause and clean up procedures. I arrived at Central Metal Finishing at approximately 4:20 p.m. and determined a copper plate strip tank had been melted and eventually caught fire due to a faulty heater within the tank. I supplied the fire department with a MSDS of the chemical content of the tank (sodium cyanide based) and proceeded to wet vacuum the contents of the spill on the plating floor of approximately 20 gallons. The spill was completely contained to the. plating floor and did not spread beyond the immediate vicinity of the strip tank. The Emergency Response Coordinator and two additional Lab Technicians arrived at the scene and cleaned all affected areas. Protective Tyvek clothing was used in conjunction with rubber boots and gloves by all responding personnel. Articles of clothing were disposed of and all boots and gloves were thoroughly rinsed after response to the .spill. The North Andover- Fire Department stayed at Central until all questions had been answered and pertinent information was revealed and satisfactory cleanup had taken place.
