No disturbance of existing grade, soils or vegetation is permitted in the No-Disturbance zone. 33.There shall be no increase in the post development discharges from the storm drainage system or any other changes in post development conditions that alter the post development watershed boundaries as currently depicted in the Notice of Intent and approved by this Order of Conditions, unless specifically approved in writing by the Commission. 34.This document shall be included in all construction contracts, subcontracts, and specifications dealing with the work proposed and shall supersede any conflicting contract requirements. The applicant shall assure that all contractors, subcontractors, and other personnel performing the permitting work are fully aware of the permits terms and conditions. By acceptance of this Order, the owners agree to indemnify and hold harmless to the Town and its residents for any damage attributable to alterations undertaken on this property pursuant to the Order. Maintenance of the drainage system, if accepted by the Town as part of a public way, becomes the responsibility of the Town. 36.The NACC finds the applicant's proposal for 360 sq. ft. of wetland restoration to be adequate for the 180 sq. ft. encroachment into the 25' No Disturbance Zone on lot 2. 37. Issuance of these Conditions does not in any way imply or certify that the site or downstream areas will not be subject to flooding, storm damage or any other form of water damage. The following information provided is to be used as a guideline for monitoring and maintaining the performance of the drainage facilities and to ensure that the quality of water runoff meets the standard set forth by the Policy. The Stormwater Management System shall be inspected during rainfall conditions during the first year of operation to verify functionality. BMPs included in the design consist of theuse of; deep sump catch basins with hoods inside grass lined treatment swales rip rap at pipe outlets extended detention basins vegetative growth within the detention basins, specific maintenance schedule restrictions on the use of Pesticides and Herbicides with in the 100 foot buffer MAINTENANCE AND MONITORING of BMPs Catch Basins Catch basin grates shall be checked monthly and following heavy rainfalls to verify that the inlet openings are not clogged by debris. Debris shall be removed from the grates and disposed of properly. Deep sump catch basins shall be inspected and cleaned semi- annually of all accumulated sediments. Catch basins with hoods shall be inspected quarterly to check oil build-up and outlet obstructions. Material shall be removed from catch basins and disposed of in accordance with all applicable regulations. Detention Basins Detention basins shall be inspected at least once per year to ensure that the basins are operating as intended and immediately after heavy rainfall events to verify that outlets and inlets are not blocked by litter or other debris. Accumulated debris shall be removed as soon as possible. Basin slopes and embankments shall be'checked for signs of erosion. Eroded slopes shall be stabilized promptly and repaired to eliminate future deterioration of the structure. Deeply accumulated sediments shall be removed from the basin periodically to ensure proper functioning of the structure. Vegetative growth shall be monitored and maintained as needed. Tree and brush growth on dikes shall be removed to maintain the structural integrity of the embankment bi-annually. Outlet Structures All outfall protection structures shall be inspected semi-annually and following major storm events to check for signs for erosion. Any necessary repairs shall be performed promptly. All outlet protection structures shall be inspected twice per year and cleaned to remove accumulated sediment as necessary. Rip-Rap overflow structure shall be weeded and cleaned on a semiannually basis to ensure that water overflowing the spillway will not become obstructed by debris. Water Quality Swales Water quality swales shall be checked for debris accumulation on a semi-annually basis. Additional inspections should be scheduled during the first few months to make sure that the vegetation in the swales becomes adequately established. Trash, leaves, branches, etc. shall be removed from channel areas. Silt, sand and sediment, if significant accumulation occurs, shall be removed by hand annually. Care shall be taken to maintain vegetation growth within a swale. Grass shall be cut and weeds and brush removed or trimmed at regular intervals during the growing season. Reseeding and weed control may need to be performed periodically to maintain healthy, dense vegetation and maintain the pollutant removal efficiency of the Swale. Any channel erosion within swales shall be stabilized and repaired as soon as practical_ Street Maintenance Paved areas shall be swept by vacuum sweepers periodically during dry weather to remove excess sediments, reducing the amount of sediments that the drainage system shall have to remove from the runoff. Salt used for de-icing on the roadway during winter months should be limited as much as possible as this will reduce the need for removal and treatment. However, Difficulties t may arise in the enforcement of such restrictions. Sand containing the minimum amount of calcium chloride (or approved equivalent) needed for handling may be applied as part of the routine winter maintenance activities. Pesticides, Herbicides and Fertilizers Pesticides and herbicides shall not be used within the limits 100-foot buffer. In addition, fertilizers that are used within this zone should be restricted to the use of organic fertilizers only. MAINTENANCE RESPONSIBILITIES All post construction maintenance activities will be documented and kept on file and made available to the Conservation Commission upon request. All post construction maintenance activities shall survive the Order of Conditions and shall run with the title of the property. All structural BMPs within the proposed roadway and easements as identified on the site plans will be owned and maintained by the developer until such a time that an association is created. The Campbell Forest cul-de-sac layout is located off of Webster Woods Lane. The new cul-de-sac will service a total of 6 lots. There are currently 5 approved lots in this portion of the subdivision. Lot#5 is currently accessed via Webster Woods Lane. Lots#144 are accessed via two private driveways extending from Webster Woods Lane a distance of approximately 480 feet. We are proposing to consolidate the private driveways into one public road and reconfigure the lots in order to create one additional lot. This allows us the opportunity to design these lots on a public right-of-way and provide a more direct access to each lot. I would respectfully request being put on the agenda to meet with the Technical Review Committee(TRC) at your earliest convenience. I would appreciate a call notifying me of the time and location of this meeting. I will notify my engineer and my client so that we may all attend. Once we have the comments from the TRC,we would Iike to meet with the planning board soon thereafter.
