I would like a one year extension on my sewage disposal system permit No. 525. I would
also like the date changed from May 4, 1992 to the date of final approval which was in
September 1992( see receipt for design approval resubmit).
I have enclosed a copy of the permit and last resubmit receipt. Please let me know what I
must do to get this extension.
