Date : July 23, 1996 At 1050 hrs E-1 responded to Box 1433 (Crusader Paper)for the second time in less than one hour. Both times the indicator panel showed sprinkler zone activation. As the previous time we found no water moving through the sprinkler system. I learned through dispatch that the DPW was in the area flushing hydrants. After resetting the system E-1 crew.looked in cul de sac for water crew. We came upon a Vining garbage hauler with a dumpster container on the ground with resturant debris or liquid swill spilling around it. Upon further investigation we found a large hydralic oil spill, (estimated at 20 to 30 gals),under and around the truck. After notifying you at 1110 hrs we contacted The Board of Health, The Conservation Commission, and The State DEP. The Company that owned the truck was notified and sent a cleanup crew to handle this spill. Scott Sayer from the DEP arrived at app. 1200 hrs and contacted Vining Disposal Service management in Stoneham,to find out what their intentions were concerning this spill. They informed Scott that a cleanup crew was enroute, The crew arrived at app. 1230 hrs and cleaned the area with speedy dry and spaghnum. Scott assured me that he would follow up to insure that the cleaned up material would be disposed of properly. E-1 returned to quarters in service 1318 hrs. TEL. 682-6400 Planning Board 9-28-87 re: lot 1 Holt Rd. I Have been asked by Mr. Zanni to review the septic system plans for this lot. I compared the septic system plan by Joe Barbagallo to the site and landscape plan by The Architects Group and noticed several discrepancies. The building is offset 60 feet from the front lot line on the septic plan and 50 on the site plan. The septic plan shows barely enough breakout fill to satisfy regulations but the site plan shows a cut for a drainage swale .too close to the leach field. I recommend that the sanitary and site engineers work together to design a plan that satisfies the requirements of the Board of Health and the owner before submitting any more proposals. NELSON, I)IRE-CTOR October 26, 1987 Mr. Daniel Long, Town Clerk Town Office Building 120 Main Street North Andover, MA 01845 RE: Zanni Family Trust Site Holt Road - Site Plan Review Dear Mr. Long: The North Andover Planning Board held a Public Hearing on Monday evening, September 14 , 1987 at 8 : 00 p.m. in the Town Office Library/Conference Room upon the application of Domenick Zanni, Jr. , Dorothy Zanni, Joseph Zanni and William R. Zanni of Zanni Family Trust, 22 Lakeview Avenue, Reading, MA. The hearing was advertised' . in the North Andover Citizen on July 30 and August 6 , 1987 . The following members were present at the September 28 , 1987 meeting: Paul Hedstrom, John Burke, John Simons , Erich Nitzsche and George Perna. The petitioner seeks a Special Permit under Section 8 , Para- graph 3, Site Plan Review of the North Andover Zoning By-Law. The purpose of the Site Plan is to review the Zanni Family Trust proposal to construct an office/industrial building containing 27 , 000 square feet. The building would be constructed on a lot containing 87 , 990 square feet of land. The site is located in an Industrial-2 (1-2) Zoning District. John Simons read the legal notice to open the public hearing. The Town Planner stated that the plans were not sufficient for the Board review due to traffic circulation and drainage concerns and the applicant has been deferred until the September 28th meeting motion. At the public hearing held on September 28th, John J. Burke read the letters received from the Board of Health, Conservation Commission, the Fire Department and the Department of Public Works were read into the record. Bradley Latham, Attorney for the applicant and William P. O'Neill, the engineer for the project were interested parties . Highlights of their presentation were: 1 . Changes made to the traffic circulation around the building. 2 . Drainage plans as reviewed by the DPW. 3 . Board of Health concerns regarding the location of the septic system and the proposed grading of the site. r . Page 2 : MOTION: By John Simons to close the public � eazing and;�z the matter under advisement. SECOND: George Perna VOTE: Unanimous On October 5, 1987 , the Planning Board held a regular meeting. The Board considered a memo written by the Town Planner which recommended that the Board approve the Special Permit application by the Zanni Family Trust with the following conditions: 1 . That two additional catch basins be placed in the parking areas to the rear of the structure and be tied into the drainage system. 2 . That the spetic system be designed and constructed according to the Board of Health Regulations . Also the septic tank should not be placed over the 12" drainage pipe. 3 . Place the landscaping materials around theP erimeter of the detention pond. 4 . Thearkin s shown on P g aces p the west side of the building should be changed to parallel parking. 5. The chain link fence to be placed around the property shall be shown. 6 . The rear (South) has a 26% 9 rade. This should be stabilized with rip rap. 7 . The gravel indicated around the perimeter of the site adjacent to the paved areas should be a stone mulch material. 8 . The grading topos shown on the plan should be changed to reflect a higher eleveation (100 ' ) around the edge of pavement and gravel surfaces. During the meeting the Town Planner stated that the applicant has submitted a revised site plan, dated September 30 , 1987, which incorporates all of the conditions listed in the memo of October 5th. Erich Nitzsche mentioned that the backlot line has a slope of 26% . He stated that this condition should be changed to indicate a stabilized rip rap slope of 1 . 5 horizontal and 1 vertical. The Board members voted unanimously to direct the Planner to draft a decision to conditionally approve the site plan utilizing the revised, September 30, 1987 plan and Erich' s amendment. The Planning Board, duringthe October 19 1987 meeting, , � g. voted to carry over the decision on this permit to a special Board meeting to be held on October 22 , 1987 . On October 22 , 1987 the Planning Board held a meeting with the following members present and voting: Paul Hedstrom, Chairman; George Perna, Vice Chiarman; John Simons, Clerk; and Erich Nitzsche. Upon a motion by Erich Nitzsche, seconded by John Simons, the Board voted to conditionally approve the Special Permit based upon the following plans : Page 3 : Plans entitled: Site & Landscape Plan Drawing C-1 dated 24 July 1987 Revised 9/30/8.7 prepared by the Architects Group, Boston 1 . The slope of the rear property line shall be graded to a 1 .5 horizontal and 1 vertical and be stablized by rock rip-rap. 2 . The areas indicated as gravel on the Site Plan shall consist of 11" stone in a bed 8" deep. 3 . Re: Zanni Family Trust Site on Holt Rd. Dear Paul: I have reviewed the plan and would make the following comments: 1. All drainage to comply with the Board of Health rules pertaining to Septic systems relative to site drainage and town drainage. No per- mission will be given by this Department until the Board of Health confirms approval on design. Written permission must be obtained by this Department prior to starting any work. 2. All site drainage should be independent from the building and no floor or interior drains should be connected to system. 3. The drainage design of Holt Rd. drainage has an existing catch basin located as shown on plan. This basin should handle site drainage and no problems should arise as long as the grades are not altered to divert drainage flows from what they are presently. 4. I would recommend a detention and sediment ponding area be constructed at drainage discharge point. 5. Two additional drain catch basins should be added towards the rear of the building. 6. All catch basins to be standard 8" precast concrete units with four foot radius and thirty in sumps. 7. Driveway Entry Permits will be required by this Department. Yours truly, c William A.
