you of the availability of a Response Action Outcome("RAO") Statement associated with a wetland area
located south of Winter Street in North Andover, Massachusetts. This notification is being submitted'to
you in accordance with the Massachusetts Contingency Plan, 310 CMR 40.1403(f).
The RAO Report for the above-referenced property can be reviewed at the Department of Environmental
Protection,Northeast Regional Office, located at 205 Lowell Street in Wilmington,Massachusetts.

Dear Sir:
According to our records the sub-surface setaerage disposal
'systems on the above mentioned lots are in violation of Title V of the
State Sanitary Code and the North Andover Rules and Regulations for
Sewerage Disposal Systems. The violations pertain to the following:
i 1. Systems covered without final approval
2. Stone depth not sufficient on leaching fields.

3. Distribution boxes not level.
You are, therefore, requested to appear at a hearing -
before this Board at its next regularly scheduled meeting on Monday,
February 6, 1978 at 6:30 P.M. to show good and sufficient reasons
as to iAny the above-mentioned violations occurred and why the Board
of Health should not revoke your Disposal Works Installer's Permit.
