(Letter dated 1/3/17 attached). ❖ On Thursday, December 29`'' Royal Crest Maintenance Supervisor Miguel Campos accompanied by Regional Property Manager Bryan Nicholas treated all wood and exposed surfaces in Master Bathroom with X-14 cleaning product and applied HELMSMAN Spar Urethane wood sealer (MSDS sheets attached) per Board of Health Director. ❖ Royal Crest Estates Maintenance Supervisor Miguel Campos supervised the replacement of the bathroom ceiling fan motor to increase draw of fan and ensure proper moisture removal. ':• On Tuesday, January Yd a licensed plumber (Maccormack Plumbing of 17 Bridge St, #104, Billerica, MA 01821) inspected all plumbing fixtures in Master Bathroom and confirmed no leaks or issues were present in bathroom. A plumbing permit was pulled with the Town of North Andover related to the replacement of the toilet and bathroom vanity. ❖ Royal Crest Estates Maintenance installed a new cover on kitchen light fixtures. ❖ Between the dates of January 11d' and 12d' Golden Gate Services replaced walls and ceiling with moisture resistant gypsum green board. Tile walls were replaced with High Gloss Bathtub wall kit. HAZARDOUS BUILDING MATERIALS AND AIR QUALITY SPECIALISTS January 3, 2017 Ms. Milissa Titus Royal Crest Estates 50 Royal Crest Drive North Andover, MA 01845 Ref: Asbestos abatement monitoring, Unit 25-8, Royal Crest Estates, North Andover, Massachusetts Dear: Ms. Titus: Apartment Investment and Management Company (AIMCO) retained Smith and Wessel Associates, Inc. (SWA) to monitor an asbestos abatement project in Unit 25-8 at the Royal Crest Estates apartment complex located at 50 Royal Crest Drive in North Andover, Massachusetts. The area abated consisted of approximately: 200 square feet of gypsum board wall & ceiling and associated joint compound from master bathroom SWA's Massachusetts licensed Project Monitor, Ted Sherry (License # 000 1763) was on site to conduct pre -abatement inspections, area air sampling during abatement, and post abatement clearance air sampling on December 29, 2016. Golden Gates Services, Inc. (Golden Gates) of Chelsea, Massachusetts was the Massachusetts licensed Asbestos Abatement Contractor who performed the work in accordance with all applicable federal, state, and local regulations. Golden Gates prepared the work area by placing two layers of 6 -mil polyethylene sheeting over all critical barriers (i.e. windows, doors, and other openings to outside of the work area), walls and floors adhered with duct tape and spray glue as applicable. A three -stage decontamination facility was constructed contiguous to the work area and was used as the only means of entrance and egress. Negative air pressure was established in the work area relative to adjacent spaces using High Efficiency Particulate Air (TEPA) filter equipped air filtration devices (AFDs). During abatement activities, access to the work area was limited to authorized personnel only, who entered the containment area utilizing appropriate personal protective equipment (PPE) and, before exiting, followed the required decontamination procedures. Removal of asbestos -containing materials (ACM) was performed using wet methods. This involved spraying ACM with water or a solution of water and wetting agent that assisted in penetrating and saturating the ACM. After all ACM were removed, complete cleaning of the abatement area was accomplished. All waste was packaged into properly labeled 6 -mil polyethylene waste disposal bags for transport to an approved landfill. All air samples were collected onto mixed cellulose ester filters (0.8 micron pore size) in three-piece 25 -millimeter (mm) cassettes aligned open-faced. The cassettes were tilted downward at a 45° angle and placed between three and four feet above the floor. Air was drawn through the cassettes via the use of high-volume sampling pumps. Immediately before and after the sampling periods, the sampling flow rates were calibrated using a precision rotameter. The samples were analyzed on-site by phase contrast microscopy (PCM) in accordance with National Institute of Occupational Safety and Health (NIOSH) 7400 Method, A counting rules. SWA utilized the Olympus optical microscope at 400x magnification to analyze the samples. The PCM method determines the total concentration of all fibers (not exclusively asbestos) that exhibit a length to width ratio of three to one and that are at least five microns in length. SWA's Project Monitor was on site full-time during abatement activities to observe quality control and compliance with applicable state and federal regulations. All air samples collected were determined to be below the Massachusetts Department of Labor Standards (MA DLS) clearance criteria of 0.01 fibers/cc following abatement. Because successful inspection and air monitoring results were obtained, the work area was cleared for re -occupancy by all personnel. The project should not be considered complete, however, until AIMCO receives the Waste Shipment Records (WSR) documenting the proper disposal of all asbestos waste. It must be noted that even though the wall paper was removed from the bathroom walls, asbestos -containing joint compound is still present on the wall system.
